Annular Facial Sarcoidosis:A Case Report

摘　　要：患者女,65岁,右耳周、颈部皮疹不伴痛痒2年,逐渐增多扩展。皮肤科情况:右耳周、颈部见环状排列淡红色丘疹,中央轻度萎缩凹陷,伴有色素减退和色素沉着。皮损组织病理示:真皮全层内多个由上皮样细胞和多核巨细胞组成的肉芽肿,未见干酪样坏死及渐进性坏死,部分肉芽肿周围有较多淋巴细胞浸润。PAS染色及抗酸染色均为阴性。胸部CT检查示:肺纹理增多,纵膈见多发中小淋巴结影。结合皮损特征、组织病理及影像学检查结果,诊断为结节病。外用0.1%他克莫司软膏和丙酸氟替卡松乳膏7周后患者面部丘疹消退。A 65-year-old female presented with growing asymptomatic lesions without pain or itching on her right periotic and neck area for two years.The dermatological examination showed some reddish papules arranged in an annular form on the right side of her ear and neck.The center of the lesion was slightly atrophic and depressed with hyperpigmentation and hypopigmentation.Histopathological examination revealed multiple granulomas consisting of epithelioid cells and multinucleated giant cells in the whole dermis,without caseous necrosis or necrobiosis.Some foci were infiltrated by dense lymphocytes.PAS and acid-fast staining were negative.The chest CT scan showed increased lung texture and multiple small and medium-sized lymph nodes in the mediastinum.A diagnosis of sarcoidosis was established based on her characteristic skin lesions,histopathological and imaging findings.Topical 0.1%tacrolimus ointment and fluticasone propionate cream were used,and the papules resolved after the treatment of 7 weeks.
