摘 要:目的评价智能化病人自控镇痛(Ai-PCA)复合PDCA循环管理在术后镇痛中的临床效果。方法选择2022年6月至2023年5月进行择期手术并自愿使用术后镇痛泵的4560例患者,按照镇痛管理办法将其分为对照组和观察组。对照组为2022年6月至11月使用PCA镇痛的2145例患者,观察组为2022年12月至2023年5月使用Ai-PCA复合PDCA循环管理的2415例患者。比较两组的中重度疼痛发生率、随访率、不良反应发生情况、镇痛满意度及术后镇痛泵的有效单次按压率。结果观察组的中重度疼痛发生率低于对照组,随访率高于对照组(P<0.05)。观察组的恶心呕吐、头晕乏力、过度镇静发生率均低于对照组,镇痛满意度高于对照组(P<0.05)。观察组术后镇痛泵的有效单次按压率高于对照组(P<0.01)。结论Ai-PCA复合PDCA循环管理应用在术后镇痛中不仅提高了术后镇痛泵的有效单次按压率及随访率,还提高了患者的镇痛满意度和术后镇痛效果,降低了不良反应发生率。Objective To evaluate the clinical effect of artificial intelligence patient-controlled analgesia(Ai-PCA)combined with PDCA cycle management in postoperative analgesia.Methods A total of 4560 patients who underwent elective surgery and voluntarily used postoperative analgesia pump from June 2022 to May 2023 were selected and divided into control group and observation group according to the analgesic management method.The control group was 2145 patients who used PCA analgesia from June to November 2022,and the observation group was 2415 patients who used Ai-PCA combined with PDCA cycle management from December 2022 to May 2023.The incidence of moderate to severe pain,follow-up rate,adverse reactions,analgesic satisfaction and effective single compression rate of postoperative analgesic pump were compared between the two groups.Results The incidence of moderate to severe pain in the observation group was lower than that in the control group,and the follow-up rate was higher than that in the control group(P<0.05).The incidence of nausea and vomiting,dizziness,fatigue and excessive sedation in the observation group were lower than those in the control group,and the satisfaction of analgesia was higher than that in the control group(P<0.05).The effective single compression rate of postoperative analgesia pump in the observation group was higher than that in the control group(P<0.01).Conclusion The Ai-PCA combined with PDCA cycle management applied in postoperative analgesia not only improves the effective single compression rate of postoperative analgesia pump and follow-up rate,but also improves the patient's analgesia satisfaction and postoperative analgesia effect,reducing the incidence of adverse reactions.
