摘 要:目的评估多点扫描模式激光联合胰激肽原酶治疗糖尿病视网膜病变的效果。方法回顾性队列研究。分析商丘市第一人民医院2021年1月至2021年12月糖尿病视网膜病变210例(210眼)的临床资料, 按照治疗方式分为对照组(105例)和研究组(105例)。对照组患者进行胰激肽原酶治疗, 研究组患者进行胰激肽原酶联合多点扫描模式激光光凝治疗, 随访观察两组的治疗前、后最佳矫正视力(BCVA, logMAR)、黄斑中心区厚度(CMT)、血清炎症因子。结果治疗后6个月, 研究组患者BCVA为0.27±0.04, 优于对照组的0.33±0.05, 差异具有统计学意义(t=9.60,P<0.001), CMT为(242.16±26.24)μm, 低于对照组的(296.41±32.38)μm, 差异具有统计学意义(t=13.34,P<0.001);血清肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)水平分别为(10.54±2.20)、(69.68±11.30)ng/L, 低于对照组的(15.70±2.71)、(82.31±13.51)ng/L(t=15.15、7.35, 均P<0.001), 白细胞介素-10(IL-10)水平为(64.62±8.63)pg/ml, 高于对照组的(56.34±7.44)pg/ml(t=7.45,P<0.001);研究组总有效率为91.43%(96/105), 高于对照组的79.05%(83/105), 差异具有统计学意义(χ^(2)=6.40,P=0.011)。结论胰激肽原酶联合多点扫描模式激光治疗糖尿病视网膜病变患者能够提升疗效, 改善视网膜功能, 减轻机体炎症反应。Objective To evaluate the efficacy of multi-point scanning mode laser combined with pancreatic kininogenase on retinal function in diabetic retinopathy patients.Methods This was a retrospective cohort study.Clinical data of 210 eyes of 210 cases of diabetic retinopathy from Jan.2021 to Dec.2021 in the First People’s Hospital of Shangqiu City were analysed,and they were divided into the control group(105 cases)and the research group(105 cases)based on treatment methods.Patients in the control group were treated with pancreatic kininogenase,while patients in the research group were treated with pancreatic kininogenase combined with multi-point scanning mode laser photoeoagulation therapy.Preoperative and postoperative best corrected visual acuity(BCVA,logMAR),central macular thickness(CMT)and serum inflammatory factors were compared between the two groups during the follow-up.Results At 6 months after treatment,BCVA in the research group was 0.27±0.04,which was higher than that of the control group 0.33±0.05,and the difference was statistically significant(t=9.60,P<0.001).CMT was(242.16±26.24)μm.It was lower than the(296.41±32.38)μm of the control group,and the difference was statistically significant(t=13.34,P<0.001).Serum tumor necrosis factor-α(TNF-α)and interleukin-2(IL-2)levels were(10.54±2.20)ng/L and(69.68±11.30)ng/L,respectively,which were lower than those of the control group[(15.70±2.71)ng/L and(82.31±13.51)ng/L,respectively](t=15.15,7.35;all P<0.001).And the level of interleukin-10(IL-10)was(64.62±8.63)pg/ml,which was higher than the(56.34±7.44)pg/ml of the control group(t=7.45,P<0.001).The total effective rate of the study group was 91.43%(96/105),which was higher than the 79.05%(83/105)of the control group,and the difference was statistically significant(χ^(2)=6.40,P=0.011).Conclusion The combination of pancreatic kininogenase and multi-point scanning mode laser in the treatment of diabetic retinopathy can improve the curative effect,improve the retinal function,and reduce the inflamma
