从“壮火食气”探讨炎症对肺癌的影响

摘　　要：肺癌的形成不是一蹴而就,而是经过“可控性炎症-非可控性炎症-肿瘤微环境形成-肿瘤细胞增殖”这一漫长的过程,炎症在肺癌的发生发展中起重要作用。现代医学中的炎症与中医学中的“壮火”密切相关。慢性炎症和肺癌都以正气亏虚为基础,“壮火食气”是肺“炎-癌转化”的高度概括,“壮火”是其重要因素,“食气”是其根本因素,“壮火”与“食气”具有相互胶着、互相积损的病机特点,火“由少至壮再至毒”,气“由虚至损再至劳”,出现“因虚生火,由火致虚”的恶性循环,共同推动着炎症向肺癌的进展。临证时应以清“壮火”、补“虚气”为法,清“壮火”包括清实热、湿热、痰热、瘀热、癌毒等,补“虚气”包括补肺气、培土生金、金水相生等,但“虚气”不可独补,“壮火”不可孤清,应扶正与祛邪并用,补“虚气”兼以清“壮火”乃其总治则。The formation of lung cancer is not achieved overnight but through the long process of"controllable inflammation-uncontrollable inflammation-tumor microenvironment formation-tumor cell proliferation",and inflammation plays an important role in the occurrence and development of lung cancer.Inflammation in modern medicine is closely related to"Zhuang Huo(strong Fire)"in traditional Chinese medicine.Chronic inflammation and lung cancer are both based on the deficiency of Zheng Qi,and"Zhuang Huo Shi Qi(strong Fire consuming Qi)"is a high summary of the"inflammation-cancer transformation"of the lung."Strong Fire"is an important factor,while"consuming Qi"is the fundamental factor."Strong Fire"and"consuming Qi"have the pathogenesis characteristics of closely related and synergetic to each other.Fire develops"from weak to strong and then to Toxin",and Qi develops"from deficiency to injury and then to exhaustion".There is a vicious circle of"Fire due to deficiency and deficiency due to Fire",which jointly promotes the progress of inflammation to lung cancer.In clinical practice,the principle of clearing"strong Fire"and relieving Qi deficiency.The former includes clearing excess Heat,Damp Heat,Phlegm Heat,stasis Heat,cancer Toxin,etc.while the latter includes replenishing Lung Qi,cultivating Earth to produce Gold as well as Gold and Water mutual producing.However,neither of the two should be conducted solely.Tonifying Zheng Qi and dispelling evil factors should be used at the same time in following the principle.
