和肝汤加减对Hp阳性消化性溃疡肝胃郁热型患者临床疗效、Hp清除率及生活质量的影响

摘　　要：目的 探讨幽门螺杆菌(Hp)阳性消化性溃疡(PU)肝胃郁热型患者采用和肝汤加减对临床疗效、Hp清除率及生活质量的影响。方法 选取2021年1月—2023年3月本院收治的76例Hp阳性PU肝胃郁热型患者,以随机数字表法分成研究组(38例)与对照组(38例),对两组临床疗效、中医证候。对照组给予常规西药治疗,研究组给予和肝汤加减治疗,治疗后对两组的积分、Hp清除率、Hp复发率、生活质量[消化系统生存质量指数(GIQLI)]进行比较。结果 研究组治疗总有效率高于对照组(P<0.05);两组治疗后中医证候积分(口干口苦、胃部疼痛、泛酸、便秘)均有降低,且研究组低于对照组(P<0.05);研究组Hp清除率高于对照组(P<0.05),Hp复发率低于对照组(P<0.05);治疗后两组GIQLI评分均有所提高(P<0.05),且研究组高于对照组(P<0.05)。结论 和肝汤加减应用于Hp阳性PU肝胃郁热型患者治疗中,可提高临床疗效,降低中医证候积分,提高Hp清除率,降低Hp复发率,促进生活质量提高。Objective To investigate the effects of Hegan Decoction on clinical efficacy,Hp clearance rate and quality of life in patients with Hp positive peptic ulcer(PU).Methods A total of 76 Hp positive PU patients with liver and stomach depression in our hospital from January 2021 to March 2023 were selected and divided into study group(38 cases) and control group(38 cases) by random number table method.The clinical efficacy of the two groups and traditional Chinese medicine syndrome control group were given conventional western medicine treatment,and the study group was given Hegan decoction plus or minus treatment.Score,Hp clearance rate,Hp recurrence rate and quality of life [digestive system Quality of Life index(GIQLI)] were compared.Results The total effective rate of the study group was 94.74%(36/38),which was higher than that of the control group 76.32%(29/38)(P<0.05).TCM syndrome score(dry mouth,bitter mouth,stomach pain,pantothenic acid,constipation) was lower in two groups after treatment(P<0.05),and the study group was lower than the control group(P<0.05).The clearance rate of Hp in the study group was higher than that in the control group(P<0.05),and the recurrence rate of Hp was lower(P<0.05).After treatment,the GIQLI score of the two groups was increased(P<0.05),and that of the study group was higher than that of the control group(P<0.05).Conclusion The application of Hegan decoction in the treatment of Hp positive PU liver and stomach depression can improve the clinical efficacy,reduce the TCM syndrome score,improve the Hp clearance rate,reduce the Hp recurrence rate,and promote the improvement of quality of life.
