摘　　要：目的 探讨胰岛素样生长因子结合蛋白7(IGFBP7)在弥漫大B细胞淋巴瘤(DLBCL)中的表达水平及与临床病理特征及预后的相关性。方法 收集2016年1月至2023年3月新疆医科大学附属肿瘤医院病理诊断为DLBCL的标本165例及其临床病理资料,采用免疫组织化学法检测IGFBP7在肿瘤组织中的蛋白表达水平,并分析其与DLBCL的临床病理特征及预后之间的关系。结果 IGFBP7表达水平与患者年龄、外周血单核细胞计数、Ki67有关(P<0.05)。多因素分析显示,患者年龄≥60岁、乳酸脱氢酶(LDH)水平≥250 U/L、累及部位为结内、外周血淋巴细胞计数降低、IGFBP7高表达是DLBCL患者预后的独立危险因素(P<0.05)。结论 IGFBP7高表达的DLBCL患者生存期相对较短,是DLBCL预后的独立危险因素,或可成为诊断DLBCL的潜在生物标志物。Objective This study aimed to investigate the expression level of Insulin like growth factor binding protein 7 in diffuse large B cell lymphoma(DLBCL)and its correlation with clinicopathological characteristics and prognosis through comprehensive analysis.Method Collected 165 specimens with pathological diagnosis of DLBCL from the hospital and their clinicopathologic data from 2016 to 2023,detected the protein expression level of IGFBP7 in tumor tissue by immunohistochemistry method,and analyzed its correlation with the clinicopathological characteristics and prognosis.Results The results showed that the expression level of IGFBP7 was related to age,peripheral blood mononuclear cell count and Ki67(P<0.05).Multivariate analysis showed that age≥60 years,lactate dehydrogenase level of the patients≥250 U/L,affected site within lymph nodes,peripheral blood lymphocyte count lower and IGFBP7 high expression level were the independent risk factors influencing the prognosis of DLBCL patients(P<0.05).DLBCL patients with high expression of IGFBP7 had a poor prognosis.Conclusion DLBCL patients with high expression of IGFBP7 have a relatively short survival time,it is an independent risk fac-tor for DLBCL and it may be a potential biomarker for the diagnosis of DLBCL.
