摘  要:目的:探讨细菌16S rRNA基因检测在新生儿败血症诊断中的应用价值。方法:选取2016年12月—2018年1月汕头大学医学院第一附属医院收治的102例疑似败血症新生儿为研究对象,其中男性66例,女性36例,年龄10 min~28 d。采集研究对象血液标本分别用血培养和细菌16S rRNA基因PCR检测法进行病原检测,选取10例同期因非感染性疾病住院的新生儿的血液标本作为对照。结果:102例疑诊败血症新生儿中的46例经临床诊断及实验室确诊为败血症,血培养阳性率32.61%(15/46),16S rRNA基因检测阳性率91.30%(42/46),16S rRNA基因检测阳性率高于血培养(P<0.001)。10例同期住院的非感染性疾病患儿的血液标本PCR检测及血培养结果均为阴性,PCR检测灵敏度为91.30%(42/46),特异度为96.43%(54/56)。结论:与传统血培养相比,16S rRNA基因PCR检测法阳性率高,且敏感准确。Objective:To investigate the value of bacterial 16S rRNA gene testing in the diagnosis of neonatal sepsis.Methods:102 suspected septic neonates admitted to the First Affiliated Hospital of Shantou University Medical College from December 2016 to January 2018 were selected for the study,including 66 males and 36 females aged 10 min-28 d.Blood specimens from the study subjects were collected for pathogen detection by blood culture and bacterial 16S rRNA gene PCR assay,respectively,and blood specimens from 10 neonates hospitalized for non-infectious diseases during the same period were selected as controls.Results:Forty-six out of 102 suspected septic neonates were diagnosed with sepsis by clinical diagnosis and laboratory confirmation,the positive rate of blood culture was 32.61%(15/46),the positive rate of 16S rRNA gene test was 91.30%(42/46),and the positive rate of 16S rRNA gene test was higher than that of blood culture(P<0.001).The results of PCR test and blood culture of 10 children with non-infectious diseases hospitalized in the same period were negative,and the sensitivity of PCR test was 91.30%(42/46),and the specificity was 96.43%(54/56).Conclusion:The bacterial 16S rRNA gene test has a high positive rate and is sensitive and accurate compared to traditional blood cultures.
