摘 要:目的:了解中国有症状人群和无症状人群芽囊原虫的流行情况及亚型分布。方法:采用Mesh主题词和自由词,包括“芽囊原虫“”人芽囊原虫“”芽囊原虫感染“”流行病学“”患病率“”发生情况“”亚型“”基因型“”中国”等在中英文数据库进行系统检索。采用Cochran's Q检验和Stata 14的I2指数进行异质性检验。结果:共纳入16项研究。芽囊原虫的分子检出率为10%(95%CI:0.07~0.12)。无症状人群芽囊原虫感染率14%(95%CI:0.08~0.19)高于有症状人群7%(95%CI:0.05~0.09)。芽囊原虫最常见的亚型为ST3,在无症状人群中感染率为7%(95%CI:0.04~0.09),在有症状人群中为3%(95%CI:0.02~0.04)。其次为ST1,在无症状人群中感染率为2%(95%CI:0.01~0.04),在有症状人群中感染率为2%(95%CI:0.01~0.03)。结论:芽囊原虫的致病性无亚型特异性,无症状人群的感染率高于有症状人群。Objective:To estimate the prevalence and subtypes distribution of Blastocystis isolated from the symptomatic and asymptomatic populations of China.Methods:A systematic search in English and Chinese databases was performed using Mesh subject and free terms,including"Blastocystis""Blastocystis hominis""Blastocystis infection""epidemiology""prevalence""occurrence""subtype(s)""subtyping""genotype(s)",and"China".The heterogeneity index was measured by estimating Cochran's Qtest and the I2 index of Stata 14.Results:In total,16 selected studies were considered for the research.The overall molecular prevalence of Blastocystis was 10%(95%CI:0.07-0.12).The prevalence of Blastocystis was higher in the asymptomatic population 14%(95%CI:0.08-0.19)than that in the symptomatic population 7%(95%CI:0.05-0.09).ST3 was identified as the most common strain of Blastocystis,showing a molecular prevalence of 7%(95%CI:0.04-0.09)in the asymptomatic population and 3%(95%CI:0.02-0.04)in the symptomatic population.ST3 was followed by ST1 with a molecular prevalence of 2%(95%CI:0.01-0.04)in the asymptomatic population and 2%(95%CI:0.01-0.03)in the symptomatic population.Conclusion:The pathogenicity of Blastocystisis not subtype-specific,and the asymptomatic population has a higher prevalence of Blastocystisinfection.
