摘 要:妊娠期糖尿病(GDM)发病率呈逐年上升趋势,该病会增加母婴心血管并发症发生风险,威胁母婴安全。GDM的发病机制尚未完全阐明,随着研究的进展,免疫炎性失调在GDM发病机制中的作用日益受到重视。GDM的发病与患者体内的慢性炎症状态有关,众多炎性因子不仅参与胰岛素抵抗(IR)致使GDM的发生,也通过损伤胰岛β细胞促进GDM的发展。本文综述与GDM发病相关的主要促炎因子白细胞介素(IL)-2、IL-6、肿瘤坏死因子-α(TNF-α)、IL-1β、干扰素-γ(IFN-γ)、C反应蛋白(CRP)、IL-17的作用机制及其与GDM发病的相关性研究,以期为GDM的防治提供参考依据。Objective The incidence rate of gestational diabetes mellitus(GDM)is increasing year by year,which will increase the risk of cardiovascular complications in mothers and infants and threaten the safety of mothers and infants.The pathogenesis of GDM has not been fully elucidated,and with the progress of research,the role of immune-inflammatory disorders in the pathogenesis of GDM is increasingly being valued.The pathogenesis of GDM is related to the chronic inflammatory state in the patient’s body.Most inflammatory factors not only participate in insulin resistance(IR),leading to GDM,but also damage the pancreatic β cells,promoting the development of GDM.This article reviews the mechanism of action of the main pro-inflammatory factors related to the pathogenesis of GDM,including interleukin(IL)-2,IL-6,tumor necrosis factor-α(TNF-α),IL-1β,interferon-γ(IFN-γ),C-reactive protein(CRP)and IL-17,and studies on their correlation with the pathogenesis of GDM,so as to provide a reference for the preve ntion and treatment of GDM.
