摘  要:目的分析江苏省1990-2019年交通伤害的疾病负担及不同危险因素的归因疾病负担情况。方法利用2019年全球疾病负担研究(GlobalBurdenofDiseaseStudy2019, GBD2019)中江苏省1990-2019年交通伤害的死亡率、早死所致寿命损失年(years of life lost, YLL)、伤残所致寿命损失年(yearslivedwithdisability, YLD)、伤残调整寿命年(disabilityadjustedlifeyears, DALY),采用Joinpoint回归分析变化趋势,并评估不同危险因素的归因疾病负担。结果1990-2019年江苏省交通事故的标化死亡率和YLL率呈下降趋势(P<0.05);标化YLD率呈上升趋势(P<0.05);标化DALY率由1 023.40/10万下降至791.70/10万[AAPC=-0.70%(-1.10%~-0.40%)],在2004年前后呈先上升后下降趋势。男性的交通事故疾病负担均高于女性,其中0~<14岁的标化死亡率(-70.87%)和标化YLL率(-71.53%)下降幅度最高;≥70岁的标化YLD率上升幅度最高(98.62%)。与1990年相比,2019年职业伤害的归因标化DALY率(123.52/10万)最高,但有所下降(-61.00%),而因饮酒和行为风险导致的归因疾病负担均有所上升,特别是归因标化YLD率。男性危险因素的归因疾病负担均高于女性。结论 江苏省因交通伤害导致的疾病负担已呈下降趋势,但仍处较高水平,未来防控重点是男性和老年人群,且随着人口老龄化加重,应继续采取科学有效的措施来降低交通伤害的疾病负担。Objective To analyze the disease burden of traffic injuries and the burden attributable to different risk factors in Jiangsu Province from 1990 to 2019.Methods Mortality,years of life lost(YLL),years lived with disability(YLD),and disability adjusted life years(DALY)due to traffic injuries in Jiangsu Province from 1990 to 2019 in Global Burden of Disease Study 2019(GBD 2019)were used to analyze the changes and trend of disease burden attributable to different risk factors using Joinpoint regression.Results From 1990 to 2019,the standardized mortality rate and YLL in Jiangsu Province due to traffic injuries showed downward trends(P<0.05);the standardized YLD showed an upward trend(P<0.05);the standardized DALY decreased from 1023.40/100000 to 791.70/100000[AAPC=−0.70%(−1.10%-−0.40%)],peaking around 2004 and declining after that.The disease burden of traffic injuries in males was higher than that in females,the standardized mortality(−70.87%)and standardized YLL(−71.53%)of the 0-<14 years age group decreased at the largest amplitude;the standardized YLD in people≥70 years old showed the largest increase(98.62%).Compared with 1990,the attributable standardized DALY(123.52/100000)caused by occupational injury in 2019 was the highest,but decreased to some extent(−61.00%),while the disease burden attributable to alcohol and behavioral risk increased,especially the attributable standardized YLD.The disease burden attributable to various risk factors was consistently higher in males than in females.Conclusions The disease burden caused by traffic injury in Jiangsu Province has shown a downward trend,but it is still at a high level.The key groups for prevention in the future are males and the elderly.With the aggravation of population aging,scientific and effective measures should be continued to reduce the disease burden of traffic injury.
