维持性血液透析患者血清Hcy、TGF-α水平变化与肾功能的相关性

摘　　要：目的分析维持性血液透析患者血清同型半胱氨酸(Hcy)、转化生长因子-α(TGF-α)水平变化与肾功能的相关性。方法选取2020年3月—2022年3月在平舆县人民医院接受维持性血液透析的糖尿病肾病患者150例作为研究组,另选取同期82名健康体检者作为对照组。比较两组(治疗前、后)血清Hcy、TGF-α水平及肾功能指标[血清肌酐(Scr)、尿素氮(BUN)、尿酸(UA)、白蛋白(Alb)]水平,分析血清Hcy、TGF-α水平变化与肾功能的相关性及联合诊断糖尿病肾病的检测价值。结果研究组治疗前、后血清Hcy、TGF-α水平高于对照组(P<0.05);研究组治疗前、后Scr、BUN、UA、Alb水平高于对照组(P<0.05);治疗前糖尿病肾病患者血清Hcy、TGF-α水平与Scr、BUN、UA水平呈正相关,与Alb水平呈负相关(P<0.05);治疗6个月后血清Hcy、TGF-α水平联合检测糖尿病肾病维持性血液透析的AUC为0.759(P<0.05)。结论维持性血液透析患者血清Hcy、TGF-α水平与肾功能水平密切相关,可作为临床诊断患者疾病转归的参考依据。Objective To analyze the correlation between the changes of serum homocysteine(Hcy)and transforming growth factor-α(TGF-α)and renal function in maintenance hemodialysis patients.Methods 150 diabetic nephropathy patients receiving maintenance hemodialysis in Pingyu People's Hospital from March 2020 to March 2022 were selected as the study group,and 82 healthy subjects were selected as the control group during the same period.Serum Hcy and TGF-αlevels and renal function indexes[serum creatinine(Scr),urea nitrogen(BUN),uric acid(UA),albumin(Alb)]levels of the two groups(before and after treatment)were compared,and the correlation between serum Hcy and TGF-αlevels and renal function was analyzed,and the detection value of combined diagnosis of diabetic nephropathy was analyzed.Results The serum levels of Hcy and TGF-αin the study group were higher than those in the control group before and after treatment(P<0.05).The levels of Scr,BUN,UA and Alb in the study group before and after treatment were higher than those in the control group(P<0.05).Serum Hcy and TGF-αlevels were positively correlated with Scr,BUN and UA levels,and negatively correlated with Alb levels in diabetic nephropathy patients before treatment(P<0.05).After 6 months of treatment,the AUC of serum Hcy and TGF-αcombined detection of diabetic nephropathy maintenance hemodialysis was 0.759(P<0.05).Conclusion Serum levels of Hcy and TGF-αin maintenance hemodialysis patients are closely related to renal function,which can be used as a reference for clinical diagnosis of disease outcomes in patients.
