摘　　要：胆囊癌是胆道系统中最常见的一类恶性肿瘤,根治性切除是胆囊癌患者唯一可能的治愈手段。近年来腹腔镜及机器人等微创手术在消化道肿瘤切除中得到了普遍应用,但是胆囊癌根治手术能否采用微创治疗手段仍存在一定的争议,主要集中于微创手术是否具有安全性,能否获得肿瘤完全切除,即RO切除(RO resection),能否获得足够的淋巴结清扫数目以及能否达到开腹手术的远期疗效。本文就胆囊癌的微创根治手术治疗现状及胆囊癌微创手术与开腹手术的疗效比较进行述评,并对目前存在的争议进行探讨,旨在为胆囊癌微创治疗的规范化开展提供借鉴与参考。Gallbladder cancer is the most common malignancy of the biliary tract.Radical surgery is the only curative treatment for gallbladder cancer patients.In recent years,minimally invasive surgery such as laparoscopic and robotic surgery has been widely used in gastrointestinal malignancies,but there are still some controversies about whether radical surgery of gallbladder cancer can adopt minimally invasive surgery.The controversies mainly focus on the safety of minimally invasive surgery,whether it can obtain suffi-cient R0 incisal margin,whether it can obtain sufficient number of lymph nodes dissection,and whether it can achieve the long-term efficacy of open surgery.This review summarized the current status of minimally invasive radical surgery for gallbladder cancer and com-pared the efficacy of minimally invasive surgery to open surgery,and discussed the existing disputes,aiming to provide relevant refer-ence for the standardization of minimally invasive treatment for gallbladder cancer.
