摘　　要：目的探讨ALP、GGT、IgM、TBA和CG指标在原发性胆汁性胆管炎(PBC)疾病诊断和治疗方面的应用价值。方法选取2021年1月至2022年12月期间来我院就诊的334例PBC患者作为研究对象,再按照AMA-M2抗体阳性强度分为AMA弱阳性组106例和阳性组228例。并选择同期AMA-M2阴性的表观健康者171例作为对照组。分别留取血清样本用于各指标的检测。各项数据使用SPSS进行统计学分析。结果AMA-M2阳性组ALP、GGT、IgM、TBA和CG的血清浓度水平及异常率均高于对照组(P<0.01),且ALP和GGT两项目的血清水平也高于AMA-M2弱阳性组(P<0.05)。而AMA-M2弱阳性组IgM的浓度水平和异常率,以及TBA的异常率均显著高于对照组(P<0.01)。此外,通过二元Logistic回归分析可知IgM还是PBC的独立危险因素。结论ALP、GGT、IgM、TBA和CG在PBC疾病进展、病因研究和早期诊断方面具有潜在的临床价值。Objective To explore application values of ALP,GGT,IgM,TBA and CG indexes in the diagnosis and treatment of PBC.Methods A total of 334 patients with PBC who visited our hospital from January,2021 to December,2022 were selected as the study objects,and then divided into two study groups:106 patients in the weak AMA positive group and 228 patients in the positive group according to the positive intensity of AMA-M2 antibody.In the meantime,a total of 171 apparent healthy subjects with negative AMA-M2 were selected as the control group.Serum samples were collected for the detection of each index among all subjects.SPSS was used for all statistical analysis.Results Serum concentrations and abnormal rates of ALP,GGT,IgM,TBA and CG in AMA-M2 positive group were higher than those in the control group(P<0.01),and serum levels of ALP and GGT were also higher than those in AMA-M2 weak positive group(P<0.05).The IgM concentration and abnormal rate and TBA abnormal rate of AMA-M2 weak positive group were significantly higher than those of the control group(P<0.01).In addition,binary logistic regression analysis showed that IgM was an independent risk factor for PBC.Conclusion Our results indicate that ALP,GGT,IgM,TBA and CG have potential clinical values in disease progression,etiology and the early diagnosis of PBC.
