整体分层针刀松解术联合骨伤按摩手法治疗腰椎间盘突出症疗效观察

摘　　要：目的 观察整体分层针刀松解术联合骨伤按摩手法治疗腰椎间盘突出症(LDH)的临床疗效。方法 选取2019年3月—2021年3月广东省中西医结合医院诊治的380例LDH患者作为研究对象,按照“掷硬币”随机法分为观察组192例与对照组188例。对照组予以骨伤按摩手法治疗,1次/d,4周为1个疗程。观察组予先予以骨伤按摩手法治疗,其操作同对照组,在第1、5、9天予以整体分层针刀松解术治疗。对比2组疗效,治疗前后腰椎活动度、中医证候积分、视觉模拟量表(VAS)评分。结果 观察组优良率高于对照组(P<0.05)。治疗后,2组腰椎前屈、后伸活动度均较治疗前升高(P<0.05),且观察组腰椎前屈、腰椎后伸活动度高于对照组(P<0.01);2组中医证候总积分均较治疗前降低(P<0.05),且观察组中医证候总积分低于对照组(P<0.01);2组VAS评分均较治疗前降低(P<0.05),且观察组VAS评分低于对照组(P<0.01)。结论 整体分层针刀松解术联合骨伤按摩手法治疗LDH疗效显著,可显著改善腰椎功能及临床症状,减轻疼痛。Objective To investigate the curative effect of holistic stratified needle-knife releasecombined with bone injury massage on lumbar disc herniation(LDH).Methods A total of 380 LDH patients diagnosed and treated from March 2019 to March 2021 in Guangdong Integrated Traditional Chinese and Western Medicine Hospital were selected as the study objects.According to the random coin toss method,they were divided into the observation group(n=192),who were given the holistic stratified needle-knife release combined with bone injury massage once a day with 4 weeks as one course,and the control group(n=188),who were given the bone injury massage as same as the observation group.On the 1st,5th and 9th day,the whole layered needle-knife release was used.Lumbar motion,TCM syndrome score and visual analog scale(VAS)score were compared before and after treatment.Results The excellent and good rate of the observation group was higher than that of the control group(P<0.05).After treatment,the flexion and extension of the lumbar spine in both groups were increased(P<0.05),and the flexion and extension of the lumbar spine in the observation group were higher than those in the control group(P<0.05).After treatment,the total scores of TCM syndromes in the two groups were lower than those before treatment(P<0.05),and the total scores of TCM syndromes in the observation group were lower than those in the control group(P<0.05).After treatment,the VAS scores of the two groups were lower than those before treatment(P<0.05),and the VAS scores of the observation group were lower than those of the control group(P<0.05).Conclusion The holistic stratified needle-knife release combined with bone trauma massage has a significant effect in the treatment of LDH,and it can significantly improve lumbar function and clinical symptoms,and relieve pain.
