摘 要:乌头碱是乌头属植物中最主要的毒性成分。乌头碱及乙醇中毒后均可引起心脏毒性,进而诱发心律失常。乌头碱和乙醇中毒致心律失常机制方面有较多共同点,如改变心肌细胞离子通道、影响心肌细胞能量代谢、诱发氧化应激和降低心肌间隙连接蛋白表达。临床工作中常见饮酒并食用乌头属植物或饮用乌头类泡酒中毒引起心律失常的患者,但目前其致心律失常的机制尚不明确。分析探讨乌头碱-乙醇中毒致心律失常机制的共同之处,可为两者联合中毒的救治提供参考。Aconitine is the most toxic component in aconitum.Both aconitine and alcohol poisoning can cause cardiac toxicity,and then induce arrhythmia.The mechanism of arrhythmia caused by aconitine and ethanol poisoning has many similarities,such as changing the ion channel of cardiomyocytes,affecting the energy metabolism of cardiomyocytes,inducing oxidative stress and reducing the expression of myocardial connexin.Patients with arrhythmia induced by aconitine combined with alcohol intake or aconitum wine poisoning are often encountered in clinical work,but the mechanism of thus induced arrhythmia is still unclear.The analysis and discussion on the common mechanism of arrhythmia caused by aconitine-ethanol intoxication can provide reference for the treatment of the combined intoxication.
