不同子宫全切术式对子宫肌腺症患者应激反应及微炎症因子的影响

摘　　要：目的:探讨不同子宫全切术式对子宫肌腺症患者应激反应及微炎症因子的影响.方法:挑选2020年1月至2023年1月我院治疗的94例子宫肌腺症患者作为对象.所有患者根据不同手术方式分为TAH组(n=44)和LTH组(n=50).TAH组采用开腹全子宫的切除术(Transabdominal hysterectomy,TAH)治疗;LTH组采用腹腔镜下的全子宫切除术(Laparoscopic total hysterectomy,LTH)治疗.分析比较两组围手术期相关指标、应激反应、微炎症因子指标及术后疼痛情况.结果:LTH组术中具体的出血量、手术操作具体的时长、术后排气具体的时长、术后住院具体的天数均比TAH组低(P<0.05).LTH组术后3 d应激反应指标皮质醇(Corticosteroids,COR)、血管紧张素(AngiotensinⅡ,AngⅡ)、肾上腺素(Epinephrine,E)水平均比TAH组低(P<0.05).LTH组术后3 d微炎症因子指标降钙素原(Procalcitonin,PCT)、白细胞介素6(Interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)水平均比TAH组低(P<0.05).LTH组术后12 h、术后1 d及术后3 d疼痛评分均比TAH组低(P<0.05).结论:与TAH术式相比,LTH术式存在创伤小、出血少、术后应激反应及炎症反应轻、恢复快等优势.Objective:To investigate the effects of different total hysterectomy on stress response and microinflammatory factors in patients with myoadenosis.Methods:94 patients with myoadenosis treated in our hospital from January 2020 to January 2023 were selected as subjects.All patients were divided into TAH group(n=44)and LTH group(n=50)according to different surgical methods.The TAH group was treated with Transabdominal hysterectomy(TAH).The LTH group was treated by Laparoscopic total hysterectomy(LTH).The perioperative indexes,stress response,microinflammatory factors and postoperative pain were analyzed and compared between the two groups.Results:The specific amount of blood loss,the specific time of operation,the specific time of postoperative exhaust and the specific days of postoperative hospitalization in LTH group were lower than those in TAH group(P<0.05).The levels of Corticosteroids(COR),AngiotensinⅡ,AngⅡand Epinephrine E in the LTH group were lower than those in the TAH group(P<0.05).The levels of Procalcitonin(PCT),Interleukin-6(IL-6)and C-reactive protein(CRP)in the LTH group were lower than those in the TAH group(P<0.05).Pain scores 12 hours,1 day and 3 days in LTH group were lower than those in TAH group(P<0.05).Conclusion:Compared with TAH operation,LTH operation has the advantages of less trauma,less bleeding,less postoperative stress and inflammation reaction,and faster recovery.
