摘 要:目的:探讨甲状腺功能正常的2型糖尿病(T2DM)患者促甲状腺激素水平与体脂含量和脂质代谢的关系。方法:回顾性分析2021年6月—2023年1月在本院内分泌科住院治疗的100例被诊断肥胖且甲状腺功能正常的2型糖尿病患者的临床资料,按照TSH水平将患者分为3组,即31例低水平组(0.34~1.75μIU/mL)、34例中水平组(1.76~3.50μIU/mL)和35例高水平组(3.51~5.60μIU/mL)。比较不同水平的TSH与体脂含量和脂质代谢的差异,应用Pearson相关检验及多因素线性回归模型进行数据分析。结果:在甲状腺功能正常的T2DM患者中,TSH相对高水平组体重指数(BMI)、糖化血红白蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)、脂联素(Adiponectin)、瘦素(Leptin)、内脂素(Visfatin)、身体脂肪百分比(BFP)和内脏脂肪含量(VFC)显著高于低水平组(P<0.05),Pearson相关检验发现TSH与BFP、VFC、Adiponectin、Leptin、Visfatin呈正相关,多元逐步回归分析发现BFP、VFC、Adiponectin、Leptin、Visfatin是影响TSH的独立危险因素。结论:促甲状腺激素的变化与甲状腺功能正常的T2DM患者体内各种脂肪含量和脂质代谢有关。Objective:To investigate the relationship between thyroid stimulating hormone(TSH)level and body fat content and lipid metabolism in type 2 diabetes patients with normal thyroid function.Methods:The clinical data of 100 patients with type 2 diabetes diagnosed as obese and with normal thyroid function who were hospitalized in the Department of Endocrinology of our hospital from June 2021 to January 2023 were retrospectively analyzed.The patients were divided into three groups according to the level of TSH,namely 31 cases in low level groups(0.34~1.75μIU/mL),34 cases in the medium level group(1.76~3.50μIU/mL)and 35 cases in high-level groups(3.51~5.60μIU/mL).Compare the differences between different levels of TSH and body fat content and lipid metabolism,and use Pearson correlation test and multivariate linear regression model for data analysis.Results:In newly diagnosed T2DM patients with normal thyroid function,the relatively high levels of TSH group included body mass index(BMI),glycosylated red albumin(HbA1c),insulin resistance index(HOMA-IR),Adiponectin,Leptin,and Visfatin.Body fat percentage(BFP)and visceral fat content(VFC)were significantly higher than those in the low level group(P<0.05).Spearman correlation test found that TSH was positively correlated with BFP,VFC,adiponectin,leptin,and visfatin.Multiple stepwise regression analysis found that BFP,VFC,adiponectin,leptin,and visfatin were independent risk factors affecting TSH.Conclusion:The changes in thyroid stimulating hormone are related to various fat contents and lipid metabolism in T2DM patients with normal thyroid function.
