摘　　要：目的 探讨血红蛋白与红细胞分布宽度(RDW)比(HRR)、糖类抗原19-9(CA19-9)及癌抗原125(CA125)与子宫内膜异位症严重程度的相关性及诊断价值。方法 选取2020年1月—2021年12月在南昌大学第一附属医院确诊为子宫内膜异位症的患者280例为研究组,选取同期健康女性280例为对照组。收集血清CA125、CA19-9及血常规结果,分析外周血细胞参数、血细胞衍生参数、血清肿瘤标志物在健康女性与子宫内膜异位症患者中的差异,评估差异指标与子宫内膜严重程度的关系。应用受试者工作特征(ROC)曲线分析比较三者以及联合指标(HRR+CA125+CA19-9)对子宫内膜异位症的诊断效能。结果 研究组患者的HRR水平[9.41(7.68~10.41)]低于对照组[10.47(9.84~10.99)],CA125水平[54.25(22.33~67.66)U/ml]高于对照组[19.09(11.60~20.29)U/ml],CA19-9[34.40(11.05~43.51)U/ml]高于对照组[11.71(6.20~15.70)U/ml],差异均有统计学意义(均P<0.05)。研究组中,Ⅰ~Ⅱ期为轻度组(177例),Ⅲ期为中度组(72例),Ⅳ期为重度组(31例)。轻度、中度、重度子宫内膜异位症患者HRR、CA19-9、CA125水平比较差异均有统计学意义(均P<0.05)。轻度、中度子宫内膜异位症患者中性粒细胞/淋巴细胞比值(NLR)、全身免疫炎症指数(SII)、RDW水平比较差异均有统计学意义(均P<0.05)。轻度与重度子宫内膜异位症患者血红蛋白、RDW水平比较差异均有统计学意义(均P<0.05)。ROC曲线结果表明:HRR、CA125、CA19-9以及HRR+CA125+CA19-9的ROC曲线下面积(AUC)分别为0.750(95%CI:0.709~0.790,P<0.05)、0.816(95%CI:0.780~0.852,P<0.05)、0.745(95%CI:0.704~0.786,P<0.05)和0.885(95%CI:0.807~0.876,P<0.05)。结论 HRR、CA125及CA19-9三者联合在子宫内膜异位症的诊断以及严重程度评估中具有潜在的应用价值。Objective To explore the correlations between hemoglobin to red blood cell distribution width(RDW) ratio(HRR),carbohydrate antigen 19-9(CA19-9),cancer antigen 125(CA125) and severity of endometriosis and the diagnostic value.Methods A total of 280 patients diagnosed as endometriosis in the First Affiliated Hospital of Nanchang University from January 2020 to December 2021 were selected as study group.A total of 280 healthy women were selected as control group during the same period.The results of serum CA125,CA19-9 and blood routine examination were collected to analyze the differences of peripheral blood cell parameters,blood cell-derived parameters,serum tumor markers between healthy women and endometriosis patients,the relationships betweeen indexes and severity of endometriosis were evaluated,receiver operating characteristic(ROC) curve was used to analyze and compare the diagnostic efficacies of single detection of HRR,CA125,CA19-9,and combined detetion of the three indexes.Results HRR level in study group [9.41(7.68-10.41)] was lower than that in control group [10.47(9.84-10.99)],CA125 level in study group [(54.25(22.33-67.66)U/ml] was higher than that in control group [19.09(11.60-20.29)U/ml],CA19-9 level in study group [34.40(11.05-43.51)U/ml] was higher than that in control group [11.71(6.20-15.70)U/ml],there were statistically significant differences(P<0.05).The patients in study group were divided into three groups:177 cases in mild group(stage Ⅰ-Ⅱ),72 cases in moderate group(stage Ⅲ),and 31 cases in severe group(stage Ⅳ).There were statistically significant differences in the levels of HRR,CA19-9,and CA125 among mild,moderate,and severe group(P<0.05).There were statistically significant differences in the levels of neutrophil/lymphocyte ratio(NLR),systemic immune inflammatory index(SII),and RDW between milde group and moderate group(P<0.05).There were statistically significant differences in the levels of hemoglobin and RDW between milde group and severe group(P<0.05).ROC curve showed that the
