血清TLR4 let-7e及MCP-1在儿童变应性鼻炎中的表达及与疾病活动度的相关性

摘　　要：目的 探讨血清单核细胞球样受体4(TLR4)、let-7e及单核细胞趋化蛋白1(MCP-1)在儿童变应性鼻炎中的表达及与疾病活动度的相关性。方法 选择2021年1月—2022年10月于浙江中医药大学附属第一医院(浙江省中医院)进行治疗的86例儿童变应性鼻炎患儿设为病例组,并选择该院同期进行体检的70例健康儿童作为对照组,分析血清TLR4、let-7e及MCP-1水平变化情况及与病情程度的相关性。结果 病例组血清let-7e表达水平(0.71±0.12)较对照组(1.36±0.42)更低,TLR4及MCP-1[(56.05±7.15)ng/L、(87.84±20.54)pg/ml]表达水平较对照组[(22.41±4.84)ng/L、(60.31±18.41)pg/ml]更高,差异有统计学意义(P<0.05);轻度患儿血清let-7e(0.86±0.14)较中度、重度(0.58±0.15、0.46±0.11)患儿更高,TLR4及MCP-1[(48.56±6.78)ng/L、(80.58±20.12)pg/ml]表达水平较于中度、重度[(58.38±7.08)ng/L、(78.51±8.56)ng/L、(89.49±22.05)pg/ml、(111.08±25.16)pg/ml]患儿更低,中度患儿血清let-7e(0.58±0.15)较重度(0.46±0.11)患儿更高,TLR4及MCP-1[(58.38±7.08)ng/L、(89.49±22.05)pg/ml]表达水平较重度[(78.51±8.56)ng/L、(111.08±25.16)pg/ml]患儿更低,差异均有统计学意义(均P<0.05);相关性分析结果中显示,血清let-7e和病情程度之间呈负相关,血清TLR4及MCP-1和病情程度之间呈正相关(P<0.05)。结论 血清TLR4、let-7e及MCP-1在儿童变应性鼻炎中表达异常,与病情程度之间关系密切,对于病情控制具有重要临床意义。Objective To investigate the expression of nuclear globular receptor 4(TLR4),let-7e and monocyte chemoattractant protein 1(MCP-1) in children with allergic rhinitis and their correlation with disease activity.Methods 86 children with allergic rhinitis who were treated in the First Affiliated Hospital of Zhejiang Zhejiang Chinese Medical University(Zhejiang Hospital of Traditional Chinese Medicine) from January 2021 to October 2022 were selected as the case group,and 70 healthy children who were examined in the hospital in the same period were selected as the control group.The changes of serum TLR4,let-7e and MCP-1 levels and their correlation with the severity of the disease were analyzed.Results The expression level of serum let-7e in the case group was lower than that in the control group.The expression levels of TLR4 and MCP-1 [(56.05±7.15) ng/L,(87.84±20.54) pg/ml] in the case group were significantly higher than those in the control group(22.41±4.84) ng/L,(60.31±18.41) pg/ml.The serum let-7e of mild children was higher than that of moderate and severe children(0.58±0.15,0.46±0.11).The expression levels of TLR4 and MCP-1 [(48.56±6.78) ng/L,(80.58±20.12) pg/ml] were higher than those of moderate and severe [(58.38±7.08) ng/L,(78.51±8.56) ng/L,(89.49±22.05) pg/ml,(111.08±25) pg/ml].The expression of TLR4 and MCP-1 [(58.38±7.08) ng/L,(89.49±22.05) pg/ml] in moderate group was higher than that in severe group [(78.51±8.56) ng/L,(111.08±25.16)pg/ml].The results of correlation analysis showed that there was a negative correlation between serum let-7e and the severity of the disease,and a positive correlation between serum TLR4 and MCP-1 and the severity of the disease.Conclusion The abnormal expression of serum TLR4,let-7e and MCP-1 in children with allergic rhinitis is closely related to the severity of the disease,and has important clinical significance for disease control.
