摘 要:目的 研究采用中孕期超声参数、临床指标及早、中孕期人新饱食分子蛋白1(Nesfatin-1)和内脏脂肪组织源性丝氨酸蛋白酶抑制剂(Vaspin)浓度预测新生儿出生体质量的可行性。方法 选取2020年3—10月于广州市番禺区中心医院规律产检并分娩的资料信息完整的122例孕妇作为研究组,根据孕妇中孕期超声参数、孕妇一般特征参数、实验室检查指标以及新生儿实际出生体质量,采用多元回归分析的方法得出预测公式,选取59例无明显并发症并于该院分娩的孕妇作为验证组,验证公式的准确性。结果 逐步多元回归分析显示,中孕期腹围、中孕期B超至分娩时间差以及孕期增重与新生儿出生体质量均有相关性,Nesfatin-1和Vaspin与新生儿出生体质量无明显相关性。得出公式:EFW=-1 554.6+13.731×腹围(mm)+10.691×孕期增重(kg)+18.574×时间差(d)。根据公式预测的新生儿体质量平均值为(3 206.11±161.90)g,最大值为3 599.65 g,最小值为2 774.25 g。与实际新生儿出生体质量比较,差异无统计学意义(t=0.276,P=0.783)。根据公式计算的估计体质量以及分娩前1周内的超声估重与实际体质量相比,体质量相差≤±5%的比例分别为47.76%、48.78%,体质量相差≤±10%的比例为76.27%、80.49%,体质量相差绝对值在≤250 g的比例为61.01%、65.85%。估重准确性比较差异无统计学意义(χ^(2)=0.017、0.251、0.243,P=0.896、0.617、0.622)。结论 结合中孕期超声参数头围、腹围、中孕期B超至分娩时间差及孕妇孕期增重预测新生儿出生体质量有一定可行性。早中孕期Nesfantin-1与Vaspin的浓度与新生儿出生体质量无直接相关性。Objective To investigate the feasibility of using ultrasound parameters,clinical indicators and concentrations of nesfatin-1 and Vaspin in the first and second trimesters to predict neonatal birth weight.Methods A total of 122 pregnant women with complete data of regular prenatal examination and delivery in Panyu Central Hospital of Guangzhou from March to October in 2020 were selected as the study group.According to the ultrasonic parameters of pregnant women in the second trimester,the general characteristic parameters of pregnant women,laboratory indicators and the actual birth weight of newborns,multiple regression analysis method was used to obtain the prediction formula.Then 59 pregnant women who delivered in this hospital without obvious complications were selected as the verification group to verify the accuracy of the formula.Results According to the results of stepwise multiple regression analysis,Abdominal circumference during the second trimester,time interval between ultrasound in the second trimester and delivery,and gestational weight gain were all correlated with newborn birth weight,while Nesfatin-1 and Vaspin were not significantly correlated with newborn birth weight.The formula is as follows:EFW=-1 554.6+13.731×abdominal circumference(mm) + 10.691 × weight gain during pregnancy(kg) + 18.574 × time interval(d).The average body mass predicted by the formula was(3 206.11±161.90) g,the maximum was 3 599.65 g,and the minimum was 2 774.25 g.There was no significant difference in birth weight between the two groups(t=0.276,P=0.783).The estimated body mass calculated by the formula and the ultrasonic weight estimate during the 1 week prior to delivery were compared with the actual body mass.the difference of body mass ≤±5% was 47.76% and 48.78%,and the difference of body mass ≤±10% was 76.27% and 80.49%,respectively.The proportion of absolute difference of body mass ≤250 g is 61.01% and 65.85%.There was no significant difference in the accuracy of weight estimation(χ^(2)=0.017,0.251,0.243
