中风后呃逆治疗经验

摘　　要：呃逆是中风后常见的并发症,有的呈顽固性,多发生于脑出血、大面积梗死或者是脑干梗死,尤其是延髓梗死等疾病,提示病情危重,预后较差。中风后呃逆多持续时间长,很难自行缓解,即便短暂缓解但却容易反复发作,在一定程度上影响患者饮食、睡眠,有时会伴发消化道出血,甚至影响患者的情绪导致抑郁等,及时有效的治疗呃逆已成为临床关注的重点问题。目前临床上中风后呃逆的治疗多以西医治疗为主,但存在着不良反应多、疗效差等缺点,而中医中药治疗优点较多,比如疗效稳定、起效快、不良反应少等。笔者以临床实践为基础,通过不断跟师学习,从中药、穴位按压等方面对其中医治疗方法进行归纳整理,简要阐述了中风后呃逆的各种疗法的应用、机理及疗效,以期能够更加完善中风后呃逆中医治疗的基础理论、辨证分型及治疗方案,从而更科学系统地指导临床治疗,为今后临床治疗中风后呃逆提供参考。Hiccup is a common complication after stroke,some of which are stubborn and often occur in diseases such as cerebral hemorrhage,large-scale infarction,or brainstem infarction,especially medullary infarction,indicating a critical condition and poor prognosis.After a stroke,hiccup often lasts for a long time and is difficult to relieve on its own.Even if it is temporarily relieved,it is prone to recurrent attacks,which to some extent affects the patient's diet and sleep.Sometimes,it may be accompanied by gastrointestinal bleeding,and even affect the patient's emotions,leading to depression.Timely and effective treatment of hiccup has become a key issue of clinical concern.At present,the treatment of post stroke hiccup in clinical practice is mainly based on Western medicine,but there are many disadvantages such as adverse reactions and poor efficacy.Traditional Chinese medicine has many advantages in treatment,such as stable efficacy,fast onset,and fewer adverse reactions.Based on clinical practice and continuous learning from teachers,the author summarizes and organizes the traditional Chinese medicine treatment methods for post stroke hiccup from the aspects of traditional Chinese medicine and acupoint pressing.The author briefly explains the application,mechanism and efficacy of various therapies for post stroke hiccup,in order to further improve the basic theory,syndrome differentiation and treatment plan of traditional Chinese medicine treatment for post stroke hiccup,and to guide clinical treatment more scientifically and systematically,providing reference for future clinical treatment of post stroke hiccup.
