摘 要:目的:探讨利用氢质子磁共振波谱(hydrogen proton magnetic resonance spectroscopy,^(1)H-MRS)分析技术检测女性膀胱过度活动症(overactive bladder,OAB)患者与对照组之间膀胱壁胆碱(choline,Cho)水平的差异并探讨其潜在诊断价值。方法:选择2022年7月—2023年8月于合肥市第二人民医院诊断为OAB的女性患者(OAB组)和同时期与患者年龄、体重指数(BMI)相匹配的女性健康志愿者(对照组)。所有受试者均进行了2次膀胱多体素^(1)H-MRS扫描,分别为初始尿意产生时(膀胱充盈期)及排空膀胱后(膀胱空虚期)。通过Siemens Skyra3.0T磁共振成像系统自带后处理软件计算出膀胱前壁、后壁、两侧壁Cho/肌酸(creatine,Cr)比值,并采用t检验进行组间和组内比较。结果:本研究OAB组纳入23例,对照组纳入17例。2组进行组间比较,OAB组膀胱充盈期Cho含量较对照组显著升高(t=-19.585,P<0.01),OAB组膀胱空虚期Cho含量较对照组升高(t=-2.243,P=0.033);2组分别行组内比较,OAB组膀胱充盈期Cho含量显著高于膀胱空虚期(t=-17.989,P<0.01),对照组膀胱充盈期与膀胱空虚期Cho含量差异无统计学意义(t=-1.254,P=0.220)。2组年龄(P=0.577)、BMI(P=0.698)等指标比较差异无统计学意义。结论:^(1)H-MRS分析技术在女性OAB的诊断中可能具有较好的潜在价值。Objective:To explore the difference in bladder wall choline levels between female patients with overactive bladder(OAB)and healthy controls using hydrogen proton magnetic resonance spectroscopy(~^(1)H-MRS)technology and explore the potential diagnostic value.Methods:Female patients diagnosed with overactive bladder(OAB group)at Hefei Second People's Hospital from July 2022 to August 2023 and female healthy volunteers(control group)who matched the age and BMI of the patients during the same period were selected.All subjects underwent two multi-voxel^(1)H-MRS scans of the bladder,one when the initial urge to urinate(bladder filling period)and after emptying the bladder(empty bladder period).The Siemens Skyra 3.0T magnetic resonance imaging system's built-in post-processing software was used to calculate the choline(Cho)/creatine(Cr)ratios of the anterior,posterior and bilateral bladder walls,and the t test was used to conduct inter-group analysis.and intra-group comparison.Results:In this study,23 cases were included in the OAB group and 17 cases were included in the control group.Compared with the control group,the choline content of the OAB group during the bladder filling phase was significantly higher than that of the healthy control group(t=-19.585,P<0.01),and the choline content of the OAB group during the empty bladder phase was significantly higher than that of the healthy control group(t=-2.243,P=0.033);intra-group comparisons were made between the two groups.The choline content in the bladder filling phase of the OAB group was significantly higher than that during the bladder emptying phase(t=-17.989,P<0.01).The choline content in the bladder of the healthy control group showed no significant difference(t=-1.254,P=0.220).There was no statistically significant difference in age(P=0.577),body mass score(BMI)(P=0.698)or other indicators between the two groups.Conclusion:^(1)H-MRS analysis may have good potential value in the diagnosis of female overactive bladder.
