刍议《临证指南医案》湿热泄泻的用药规律

摘　　要：目的整理叶天士《临证指南医案·泄泻》中治疗湿热泄泻的方药,结合《神农本草经疏》进行分析探讨,总结其用药规律,以期为临床用药提供一些参考价值。方法将《临证指南医案·泄泻》中治疗湿热泄泻的方药录入Excel,统计各单味药出现的次数及其分类、归经、药性和药味。结果《临证指南医案·泄泻》中治疗湿热泄泻,11味药的使用频次最多,以利水渗湿药、补虚药为主,多归脾经,药性多偏温性,药味多为甘。结论综叶天士所言,湿困脾土、脾虚湿盛是湿热泄泻的基本病机,用药需要以健脾祛湿为主。Objective Combining Shennong’s Classic of the Materia Medica to sort out prescriptions and analyze the medication rules of the treatment for dampness-heat diarrhea in the Guide to Clinical Practice with Medical Records written by Ye Tianshi,in order to provide some reference value for clinical medication.Methods The prescriptions in dampness-heat diarrhea of the Guide to Clinical Practice with Medical Records were collected and inputted into the Microsoft Office Excel 2019 spreadsheet.The herbal frequency statistics,herbal classification,four natures,five flavors and meridian affinity were analyzed.Results There were 11 kinds of herbs appear the most.These Chinese medicines are mostly of eliminating dampness and tonifying deficiency,and belong to spleen meridian,with mild drugs property,sweet in flavor.Conclusion In Ye Tianshi’s opinion,dampness-heat diarrhea is based on the basic pathogenesis of excess dampness affecting the spleen and spleen deficiency with dampness.These results all show that the treatment principle is activating spleen and resolving dampness.
