疏肝解郁活血汤联合眼三针治疗青光眼性视神经萎缩临床观察

摘　　要：目的探讨疏肝解郁活血汤联合眼三针对青光眼性视神经萎缩患者眼血流动力学及疗效的影响。方法将102例(157眼)青光眼性视神经萎缩患者随机分为对照组(51例,77眼)和试验组(51例,80眼)。对照组给予甲钴胺,试验组在对照组基础上给予疏肝解郁活血汤联合眼三针,均治疗12周。比较两组临床疗效、视力、视野及眼血流动力学指标。结果试验组总有效率为83.75%(67/80),高于对照组的67.53%(52/77)(P<0.05)。治疗后,与对照组比较,试验组视力和视野指数(VFI)较高,视野平均缺损(MD)值较低,收缩期峰值流速(PSV)、舒张末期血流速度(EDV)较高,阻力指数(RI)较低(P<0.05)。两组不良反应发生率相比差异无统计学意义(P>0.05)。结论疏肝解郁活血汤联合眼三针能明显促进青光眼性视神经萎缩患者恢复视力,减轻其视野缺损,改善血流动力学指标,临床疗效显著。Objective To investigate the effect of Shugan Jieyu Huoxue decoction combined with eye three-needle on ocular hemodynamics and curative effect in patients with glaucomatous optic atrophy.Methods One hundred and two patients(157 eyes)with glaucomatous optic atrophy were randomly divided into the control group with 51 patients(77 eyes)and the treatment group with 51 patients(80 eyes).The control group was treated with mecobalamin,and on the basis of the control group,the treatment group was treated with Shugan Jieyu Huoxue decoction combined with eye three-needle.Both groups were treated for 12 weeks.The clinical efficacy,visual acuity,visual field and ocular hemodynamics indexes were compared between the two groups.Results After treatment,the total effective rate of the treatment group(83.75%,67/80)was higher than that of the control group(67.53%,52/77)(P<0.05).Compared with the control group,the visual acuity and visual field index(VFI)were higher,the mean visual field defect(MD)was lower,the peak systolic velocity(PSV)and end diastolic velocity(EDV)were higher,and resistance index(RI)was lower in the treatment group after treatment(P<0.05).There was no statistical difference in adverse reactions between the two groups(P>0.05).Conclusion Shugan Jieyu Huoxue decoction combined with eye three-needle can significantly promote the recovery of visual acuity in patients with glaucomatous optic atrophy,reduce visual field defect and improve hemodynamic indexes,and the clinical effect is significant.
