针刺联合通络止痛汤治疗气虚血瘀型产后腰痛临床观察

摘　　要：目的探究针刺联合通络止痛汤治疗气虚血瘀型产后腰痛(PLBP)的临床效果,以期为临床提供一种可行的治疗方案。方法选取2021年1月—2023年1月枣庄市妇幼保健院门诊明确诊断为气虚血瘀型的PLBP患者80例,按随机数字表法分为对照组40例、研究组40例。对照组和研究组均予以常规产后康复护理,对照组加用通络止痛汤治疗,研究组在对照组基础上予以针刺治疗,14 d为1个疗程,共2个疗程。观察两组临床效果以及治疗前后中医症状积分及视觉模拟量表(VAS)评分、Oswestry功能障碍指数(ODI)评分、日本骨科协会(JOA)评分。结果研究组总有效率为92.50%(37/40),高于对照组的80.00%(32/40)(P<0.05)。治疗前两组患者VAS、ODI、JOA评分比较,差异无统计学意义(P>0.05);治疗后,两组JOA评分高于治疗前,VAS、ODI评分低于治疗前,且研究组相关评分改善优于对照组(P<0.05)。治疗前两组患者中医症状积分比较,差异无统计学意义(P>0.05);治疗后,两组患者中医症状积分低于治疗前,且研究组中医症状积分明显低于对照组(P<0.05)。结论针刺联合通络止痛汤治疗气虚血瘀型PLBP具有良好的临床效果。Objective To explore the clinical efficacy of acupuncture combined with Tongluo Zhitong decoction on postpartum low back pain of qi deficiency and blood stasis type,and to provide a feasible treatment plan for clinical practice.Methods From January 2021 to January 2023,80 patients with postpartum low back pain diagnosed as qi deficiency and blood stasis type in outpatient clinics of Zaozhuang maternal and child health hospital were selected and divided into a control group of 40 cases and a study group of 40 cases according to a random number table.Both the groups received routine postpartum rehabilitation care.The control group received Tongluo Zhitong decoction treatment and the study group received acupuncture treatment on the basis of the control group,with a 14 day course of treatment.The effect and VAS,DOI,and JOA scores between the two groups were observed after two courses.Results The total effective rate of the study group was 92.50%(37/40),while that of the control group was 80.00%(32/40)(P<0.05).There was no statistically significant difference in VAS,DOI,and JOA scores between the two groups before treatment(P>0.05).After treatment,the JOA score was higher and the VAS and ODI scores were lower than those before treatment,and the improvement of related scores in the study group was better than those in the control group(P<0.05).There was no statistically significant difference in the traditional Chinese medicine symptom scores of the two groups before treatment(P>0.05).After treatment,the study group had a significantly lower overall score than the control group(P<0.05).Conclusion Acupuncture combined with Tongluo Zhitong decoction has a good clinical efficacy in the treatment of postpartum low back pain of qi deficiency and blood stasis type.
