加味调肝汤联合雌二醇片/雌二醇地屈孕酮片治疗肝郁肾虚型卵巢储备功能减退临床观察

摘　　要：目的观察加味调肝汤联合雌二醇片/雌二醇地屈孕酮片治疗肝郁肾虚型卵巢储备功能减退(DOR)的临床效果。方法选取肝郁肾虚型DOR患者60例,随机分配至对照组、治疗组,各30例。对照组给予口服雌二醇片/雌二醇地屈孕酮片治疗,治疗组给予加味调肝汤联合雌二醇片/雌二醇地屈孕酮片口服治疗。比较两组治疗前后中医证候积分、血清促卵泡生成素(FSH)、FSH/促黄体生成素(LH)、抗缪勒氏管激素(AMH)、雌二醇(E2)的变化情况。结果治疗组中医证候积分、FSH、E2改善程度优于对照组,差异有统计学意义(P<0.05);两组AMH均较治疗前升高,差异有统计学意义(P<0.05)。结论加味调肝汤联合雌二醇片/雌二醇地屈孕酮片能有效调节肝郁肾虚型DOR患者的月经状态、缓解临床伴随症状、改善生殖内分泌激素水平、升高AMH。Objective To observe the clinical effect of modified Tiaogan decoction combined with estradiol tablets/estradiol and dydrogesterone tablets in the treatment of diminished ovarian reserve(DOR)function of liver stagnation and kidney deficiency type.Methods A total of 60 DOR patients with liver stagnation and kidney deficiency were selected and randomly assigned to the control group and the treatment group,with 30 cases in each group.The control group was given oral estradiol tablets/estradiol progesterone tablets,and the treatment group was given modified Tiaogan decoction on the basis of the control group.The changes of TCM syndrome score,serum follicle-stimulating hormone(FSH),FSH/luteinizing hormone(LH),anti-Müllerian hormone(AMH),and Estradiol(E2)were compared before and after treatment.Results The improvement degree of TCM syndrome score,FSH and E2 in the treatment group was better than that in the control group,and AMH in both groups was higher than that before treatment,and the differences were statistically significant(P<0.05).Conclusion Modified Tiaogan decoction combined with estradiol tablets/estradiol progesterone tablets can effectively regulate the menstrual state,relieve clinical accompanying symptoms,improve reproductive endocrine hormone levels and increase AMH in DOR patients with liver stagnation and kidney deficiency.
