阴道三维容积超声检查对多囊卵巢综合征的诊断效果及其影像学特征分析

摘　　要：目的 探讨阴道三维容积超声(three-dimensional transvaginal ultrasound,3D-TVS)检查对多囊卵巢综合征(polycystic ovarian syndrome,PCOS)的诊断效果及其影像学特征。方法 选取2019年2月—2022年2月在古田县医院进行诊治的60例疑似PCOS患者作为PCOS组,以及同期行孕前检查的60例健康女性为对照组组。以临床综合诊断结果作为金标准,对3D-TVS检查诊断PCOS的结果实施Kappa一致性检验,判断3D-TVS诊断PCOS的敏感度、特异性和准确率。比较两组卵巢容积(ovarian volume,OV)、卵泡容积(follicle volume,FV)、卵泡数(follicle number,FN)、间质容积(stroma volume,SV)及最大卵泡直径(maximum follicle diameter,MFD),以及卵巢间质血流指标血管化指数(vascularization index,VI)、血流指数(flow index,FI)和血管化血流指数(vascularization flow index,VFI)。结果 3D-TVS诊断PCOS与临床综合诊断结果具有高度一致性(Kappa=0.792,P<0.05),诊断PCOS的敏感度、特异性、准确率分别为93.88%、90.91%和93.33%。PCOS组卵巢参数OV、FV、FN、SV指标及卵巢间质VI、FI、VFI指标均高于对照组,MFD低于对照组,差异均有统计学意义(P<0.05)。结论 3D-TVS检查对PCOS具有较高的诊断效能,能够显示卵巢结构和间质血流情况,准确测量卵巢容积、卵泡个数和大小,为POCS的诊断提供了客观定量指标,值得推广使用。Objective To explore the diagnostic efficacy and imaging features of three-dimensional transvaginal ultrasound(3D-TVS) in the diagnosis of polycystic ovary syndrome(PCOS).Methods Sixty suspected PCOS patients who underwent treatment at Gutian County Hospital from February 2019 to February 2022 were selected as the PCOS group,and 60 healthy women who underwent prenatal examinations during the same period were selected as the control group.Using clinical comprehensive diagnostic results as the gold standard,Kappa consistency test was performed to evaluate the results of 3D-TVS examination and determine the sensitivity,specificity,and accuracy of 3D-TVS in diagnosing PCOS.Compare two groups of ovarian volume(OV),follicle volume(FV),follicle number(FN),stroma volume(SV),maximum follicle diameter(MFD),as well as ovarian interstitial blood flow indicators vascularization index(VI),flow index(FI),and vascularization flow index(VFI).Results The results of 3D-TVS diagnosis and clinical diagnosis were highly consistent(Kappa=0.792,P<0.05).The sensitivity,specificity and accuracy were 93.88%,90.91% and 93.33%,respectively.The ovarian parameters OV,FV,FN,SV and the ovarian interstitial VI,FI,VFI indexes in PCOS group were significantly higher than those in control group(P <0.05),while the MFD indexes in PCOS group were significantly lower than those in control group(P <0.05).Conclusion 3D-TVS examination has high diagnostic efficacy for PCOS,which can display ovarian structure and interstitial blood flow,accurately measure ovarian volume,number and size of follicles,and provide objective quantitative indicators for the diagnosis of POCS.It is worth promoting and using.
