基于脾气散精理论辨治糖尿病干眼新思路

摘　　要：糖尿病干眼是糖尿病患者常见的眼部并发症之一,其形成与气血津液等水谷精微无法输布至目珠有关。脾气散精是气血津液代谢的关键环节。脾气散精功能的发挥依赖于丰润的脾阴濡养脾土,健运的脾阳运化水谷和畅通的脾络助脾行使机能。若脾脏阴液亏虚,湿热碍阻脾阳和痰瘀损伤脾络将引起脾气散精功能失常,气血津液等水谷精微无法输布至目珠,引发糖尿病干眼。治疗上以助脾散精为治则,辅以运脾滋阴使脾脏阴液得养,清热利湿则湿热气郁得消,化痰祛瘀则脾络痰瘀得除,旨在恢复脾气散精功能。从脾气散精理论出发,可为辨治糖尿病干眼提供新思路,从而提高临床疗效。Diabetic dry eye is one of the common eye complications in diabetic patients.Its formation is related to the inability of water and grain,such as qi-blood-body fluid,to transport to eyes.Spleen qi dispersing essence is the key link in the metabolism of qi-blood-body fluid.The function of spleen qi dispersing essence depends on the rich spleen yin to nourish the spleen soil,the healthy spleen yang to transfer water and grain,and the smooth spleen collaterals to help the spleen exercise its function.If the spleen yin fluid is deficient,dampness-heat obstructs the spleen yang and phlegm stasis damages the spleen collaterals,it will cause the function of the spleen qi dispersing essence to malfunction,and the water grain essence such as qi-blood-body fluid can not be transferred to eyes,which causic diabetic dry eye.In terms of treatment,it is to help spleen dispersing essence as the treatment principle,and transport spleen and nourish yin fluid to nourish spleen yin fluid,clearing heat and dampness to eliminate dampness-heat and qi stagnation,resolving phlegm and removing blood stasis to remove phlegm-stasis in spleen collaterals,which is to restore spleen qi dispersing essence.Based on the theory of spleen qi dispersing essence,it can provide a new idea for the differentiation and treatment of diabetic dry eye,so as to improve the clinical effect.
