基于“宛陈则除之”理论探讨瘀血型腰痛的病机及治疗

摘　　要：“宛陈则除之”源自《黄帝内经》,指凡是有血结不通表现的“宛陈”病,都可以考虑“除”之。笔者认为瘀血型腰痛主因风寒湿等邪气侵袭人体日久产生瘀血,进而痹阻气血运行,常见络脉瘀滞之处,病机主要是久病入络,本虚标实,气血不通;治当以通调气血,既要考虑风、寒、湿等病理因素,又要益气活血化瘀,可以采用刺络放血的方法,将瘀血祛除体外,加强通络之力,使气血畅通,通则不痛。文章基于瘀血与络脉之间的联系,探讨瘀血型腰痛的病因病机及治疗,从而为“宛陈则除之”理论在瘀血型腰痛中的应用提供理论依据。The theory of‘removing blood stasis'originates from the Huangdi Neijing,which means that any'Yuchen'disease with blood stasis can be considered for‘elimination'.The author believes that the main cause of blood stasis type low back pain is the invasion of pathogenic factors such as wind,cold,and dampness that invade the human body for a long time,resulting in blood stasis and obstruction of the circulation of qi and blood,and the meridian stasis is quite common.The main pathogenesis is prolonged illness entering the meridians,with deficiency in the root and excess in superficiality,and obstruction of qi and blood.When treating,it is necessary to consider pathological factors such as wind,cold,and dampness,as well as nourishing qi.promoting blood circulation,and removing blood stasis.We can use the method of puncturing the meridians and releasing blood to remove blood stasis from the body,strengthen the power of meridian circulation,and make qi and blood flow unobstructed without pain.The article is based on the relationship between blood stasis and meridians,exploring the etiology,pathogenesis,and treatment of blood stasis type low back pain,in order to provide a theoretical basis for the application of the theory of‘removing blood stasis'in blood stasis type lowback pain.
