从“胆”探讨脑小血管病病机

摘　　要：基于脑小血管病的影像学表现及临床证候等特点分析,本文依据“病厥阴风木者,肝将取决于胆”“胆主决断”“情志各异,必资胆气”“髓之虚实,肝胆应之”等古籍论述,结合当代各医家学者关于脑小血管病的辨证论治认识,进行了“胆”与脑小血管病病机相关性的初步理论探讨。笔者以为,围绕“胆木风气”“胆主决断”及“五脏情志成用”三方面的病理、生理特性阐述,能够为脑小血管病病机提供新的理论依据,为脑小血管病临床治疗提供从胆论治之新思路。Based on the analysis of the imaging manifestations and clinical syndromes of cerebral small vessel disease(CSVD),this paper is based on"patients with Jueyin Fengmu,the liver will depend on the gallbladder","the gallbladder controls the decision","different emotions,must fund the gallbladder qi","the deficiency and excess of the marrow,the liver and gallbladder should respond"and other ancient books,combined with the syndrome differentiation and treatment of CSVD by contemporary medical scholars and scholars,a preliminary theoretical discussion on the correlation between"gallbladder"and the pathogenesis of CSVD was carried out.The author believes that the elaboration of the pathological and physiological characteristics of the three aspects of"the atmosphere of the gallbladder wood","the gallbladder controls the decision"and"five zang organs and emotions"can provide a new theoretical basis for the pathogenesis of CSVD,and provide a new basis for the clinical treatment of CSVD from the gallbladder.
