超声引导下小针刀纵切腕横韧带治疗腕管综合征的效果

摘　　要：目的探讨利用可视化超声下采用小针刀纵切腕横韧带治疗腕管综合征的疗效。方法2019年6月至2021年12月选取新乡医学院第一附属医院康复科门诊收治的50例腕管综合征患者,随机分为对照组和观察组,各25例。对照组接受超声引导下单纯药物治疗,观察组接受超声引导下药物联合针刀纵切腕横韧带治疗。记录两组治疗前后视觉模拟评分法(VAS)评分,通过Kelly标准评价治疗效果。结果治疗前,两组VAS评分差异无统计学意义(P>0.05);治疗后两组VAS评分降低,且观察组低于对照组(P<0.05)。治疗后,观察组优良率高于对照组(P<0.05)。治疗前,两组豌豆骨水平正中神经横截面积差异无统计学意义(P>0.05);治疗后,两组VAS评分降低,且观察组低于对照组(P<0.05)。结论超声引导下小针刀纵切腕横韧带治疗腕管综合征的效果确切,可减轻患者疼痛。Objective To explore the effectiveness of longitudinal incision of transverse carpal ligament with small needle knife under visual ultrasound in the treatment of carpal tunnel syndrome.Methods A total of 50 patients with carpal tunnel syndrome admitted to the Rehabilitation Department of the First Affiliated Hospital of Xinxiang Medical University from June 2019 to December 2021,they were randomly divided into control group and observation group,with 25 cases in each group.The control group was treated with ultrasound-guided drug therapy,and the observation group was treated with ultrasound-guided drug combined with needle knife longitudinal incision of transverse carpal ligament.The visual analogue scale(VAS)scores of the two groups before and after treatment were recorded,and the treatment effect was evaluated by Kelly criteria.Results Before treatment,there was no statistically difference in VAS scores between the two groups(P>0.05),after treatment,the VAS scores of the two groups decreased,and the observation group was lower than the control group(P<0.05).After treatment,the excellent and good rate of the observation group was higher than that of the control group(P<0.05).Before treatment,there was no statistically difference in the cross-sectional area of the median nerve at the pea bone level between the two groups(P>0.05),after treatment,the VAS scores of both groups decreased,and the observation group was lower than the control group(P<0.05).Conclusion The ultrasound-guided small needle knife longitudinal resection of the transverse carpal ligament has a definite effect on the treatment of carpal tunnel syndrome and can alleviate patient pain.
