摘  要:新生儿溶血性黄疸是新生儿病理性黄疸的重要原因之一,诊断不及时容易并发核黄疸或其他神经功能障碍,早期及时识别及监测新生儿溶血性黄疸的严重程度非常重要。矫正室内一氧化碳的呼气末一氧化碳值(end-tidal carbon monoxide corrected for inhaled carbon monoxide,ETCOc)可以无创、快速、床边测量溶血性黄疸新生儿体内胆红素产生,可定量测定溶血率,假阳性和假阴性少;ETCOc在新生儿溶血性黄疸的早期诊断和评估严重程度、预测住院天数均有一定的价值,联合直接抗球蛋白试验在新生儿ABO溶血病的精确诊断中发挥重要作用。ETCOc作为一种无创持续评估患儿病情变化的检测方法能够有效监测溶血程度、并能协助及早期诊断新生儿溶血性黄疸,以指导临床医生采取相应措施,减少新生儿溶血性黄疸的后遗症,值得在临床推广。Neonatal hemolytic jaundice is one of the significant causes of pathological jaundice in newborns.Failure to diagnose it promptly can lead to complications such as kernicterus or other neurological impairments.Early identification and monitoring of the severity of neonatal hemolytic jaundice are crucial.Using the end-tidal carbon monoxide corrected for inhaled carbon monoxide(ETCOc)as a measure can non-invasively and rapidly measure bilirubin production in hemolytic jaundice neonates at the bedside.It can quantitatively determine hemolysis rate with minimal false positives and false negatives.ETCOc has certain application value in the early diagnosis and assessment of the severity of neonatal hemolytic jaundice,predicting length of hospital stay,and plays an important role in the accurate diagnosis of neonatal ABO hemolytic disease when combined with direct antiglobulin tests.ETCOc helps in the early diagnosis of neonatal hemolytic jaundice and serves as a non-invasive method for continuously assessing changes in the children’s condition,effectively assisting neonatologists in monitor the degree of hemolysis regularly and taking appropriate clinical measures to avoid further adverse consequences and reduce the sequelae of neonatal hemolytic jaundice.Therefore,it is expected to be widely carried out in clinical practice.
