摘　　要：骨质疏松性肱骨近端骨折(OPHF)是老年人常见骨质疏松骨折之一,发生率仅次于椎体压缩性骨折、髋部骨折和桡骨远端骨折。OPHF患者骨质量差、骨折粉碎、愈合缓慢,严重影响肩关节功能,临床诊疗困难。在OPHF的诊断、治疗和康复方面,中西医累积了丰富经验和循证医学证据,取得良好效果,但如何结合两种体系优势协同诊疗尚缺乏相关共识的指导。为促进OPHF中西医协同诊疗,由中国老年学和老年医学学会老年病学分会骨科专家委员会、中华医学会骨科学分会青年骨质疏松学组、中国医师协会骨科医师分会骨质疏松学组和上海中西医结合学会骨质疏松专业委员会组织有关专家,基于循证医学证据,制订《骨质疏松性肱骨近端骨折中西医协同诊疗专家共识(2024版)》,就OPHF的中西医协同诊断、治疗与康复方面提出13条推荐意见,旨在促进OPHF中西医协同诊疗规范化、系统化、个性化,改善患者功能。Osteoporotic proximal humeral fracture(OPHF)is one of the common osteoporotic fractures in the aged,with an incidence only lower than vertebral compression fracture,hip fracture,and distal radius fracture.OPHF,secondary to osteoporosis and characterized by poor bone quality,comminuted fracture pattern,slow healing,and severely impaired shoulder joint function,poses a big challenge to the current clinical diagnosis and treatment.In the field of diagnosis,treatment,and rehabilitation of OPHF,traditional Chinese and Western medicine have accumulated rich experience and evidence from evidence⁃based medicine and achieved favorable outcomes.However,there is still a lack of guidance from a relevant consensus as to how to integrate the advantages of the two medical systems and achieve the integrated diagnosis and treatment.To promote the diagnosis and treatment of OPHF with integrated traditional Chinese and Western medicine,relevant experts from Orthopedic Expert Committee of Geriatric Branch of Chinese Association of Gerontology and Geriatrics,Youth Osteoporosis Group of Orthopedic Branch of Chinese Medical Association,Osteoporosis Group of Orthopedic Surgeon Branch of Chinese Medical Doctor Association,and Osteoporosis Committee of Shanghai Association of Integrated Traditional Chinese and Western Medicine have been organized to formulate Expert consensus on the diagnosis and treatment of osteoporotic proximal humeral fracture with integrated traditional Chinese and Western medicine(version 2024)by searching related literatures and based on the evidences from evidence⁃based medicine.This consensus consists of 13 recommendations about the diagnosis,treatment and rehabilitation of OPHF with integrated traditional Chinese medicine and Western medicine,aimed at standardizing,systematizing,and personalizing the diagnosis and treatment of OPHF with integrated traditional Chinse and Western medicine to improve the patients′function.
