二甲双胍联合克罗米芬治疗多囊卵巢综合征致不孕症疗效及对FSH、FINS、HOMAIR的影响

摘　　要：目的探讨多囊卵巢综合征致不孕症患者治疗采用二甲双胍联合克罗米芬的临床疗效和对性激素、胰岛功能的影响。方法选取90例多囊卵巢综合征致不孕症患者,基于随机数字表法分为对照组与研究组,每组45例。对照组应用克罗米芬治疗,研究组应用克罗米芬联合二甲双胍治疗。比较两组的临床疗效、生化指标变化和妊娠结局。结果研究组治疗总有效率93.33%高于对照组的77.78%(P<0.05)。与治疗前比较,两组治疗后卵泡刺激素(FSH)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMAIR)指标均降低,且研究组明显低于对照组(P<0.05)。研究组妊娠率、排卵率高于对照组,成熟卵泡个数多于对照组(P<0.05),两组早孕流产率比较差异无统计学意义(P>0.05)。结论多囊卵巢综合征致不孕症患者采用二甲双胍联合克罗米芬治疗,可改善机体胰岛素抵抗,促进正常排卵,实现提高妊娠率的治疗目的,具有临床推广价值。Objective To in vestigate the clinical efficacy of metformin combined with clomiphene in the treatment of infertility caused by polycystic ovary syndrome(Pcos)and the effects on sex hormone and islet function.Methods 90 patients with infertility caused by PCOS were selected and divided into control group and study group based on random number table method,with 45 cases in each group.The control group was treated with clomiphene,and the study group was treated with clomiphene combined with metformin.The clinical efficacy,biochemical index changes and pregnancy outcome of the two groups were compared.Results The total effective rate of the study group(93.33%)was higher than that of the control group(77.78%)(P<0.05).Compared with before treatment,the indexes of follicle stimulating hormone(FSH),fasting insulin(FINS)and insulin resistance index(HOMAIR)in both groups were decreased after treatment,and the study group was significantly lower than the control group(P<0.05).The pregnancy rate and ovulation rate in the study group were higher than those in the control group(P<0.05),and the number of mature follicles in the study group was higher than that in the control group(P>0.05).There was no significant difference in the rate of early pregnancy abortion between the two groups(P>0.05).Conclusion The combination of metformin and clomiphene in the treatment of infertility caused by polycystic ovary syndrome can improve insulin resistance,promote normal ovulation,and achieve the therapeutic purpose of increasing pregnancy rate,which has clinicalvalue.
