经阴道彩色多普勒超声联合血清孕酮、人绒毛膜促性腺激素对早期先兆流产的诊断及预后评估价值

摘　　要：目的 探讨经阴道彩色多普勒超声联合血清孕酮、人绒毛膜促性腺激素对早期先兆流产的诊断及预后评估价值。方法 选取本院诊治的100例先兆流产孕妇为研究对象,按照预后情况分为先兆流产妊娠继续50例(A组),先兆流产妊娠失败50例(B组),另选取同期50例正常妊娠孕妇为对照组,比较三组的经阴道彩色多普勒超声诊断结果及血清孕酮、人绒毛膜促性腺激素水平。结果 A、B组血清孕酮、人绒毛膜促性腺激素水平低于正常组,B组血清孕酮、人绒毛膜促性腺激素水平低于A组,差异具有统计学意义(P<0.05);A组卵黄囊全部检出,卵黄囊直径均<7 mm,妊娠囊位置正常,宫腔内见不规则液性暗区6例;B组卵黄囊检出41例,卵黄囊直径<7 mm 39例、≥7 mm 11例,妊娠囊位于宫腔中下段3例,妊娠囊变形5例,宫腔内见不规则液性暗区29例。结论 经阴道彩色多普勒超声联合血清孕酮、人绒毛膜促性腺激素对早期先兆流产具有较高的诊断价值,可有效指导临床决策。Objective To explore the diagnostic and prognostic value of transvaginal color doppler ultrasound combined with serum progesterone and human chorionic gonadotropin for early threatened miscarriage.Methods 1oo pregnant women with threatened miscarriage treated in our hospital were selected as the research subjects.According to the prognosis,they were divided into 50 cases of continued threatened miscarriage(Group A)and 50 cases of failed threatened miscarriage(Group B).In addition,50 normal pregnant women during the same period were selected as the control group.The diagnostic results of transvaginal color doppler ultrasound and serum progesterone and human chorionic gonadotropin levels were compared among the three groups.Results The serum progesterone and human chorionic gonadotropin levels in groups A and B were lower than those in the normal group,while the serum progesterone and human chorionic gonadotropin levels in group B were lower than those in group A,with statistical significance(P<0.05);All yolk sacs in group A were detected,with a diameter of less than 7 mm.The position of the gestational sac was normal,and irregular liquid dark areas were observed in 6 cases within the uterine cavity;41 cases of yolk sac were detected in group B,including 39 cases with yolk sac diameter<7 mm and 11 cases with yolk sac diameter≥7 mm,There were 3 cases of gestational sac located in the middle and lower part of the uterine cavity,5 cases of gestational sac deformation,and 29 cases of irregular liquid dark area in the uterine cavity.Conclusion Transvaginal color doppler ultrasound combined with serum progesterone and human chorionic gonadotropin has high diagnostic value for early threatened miscarriage,and can effectively guide clinical decision-making.
