子宫压迫缝合联合宫腔球囊压迫在中央性前置胎盘孕妇剖宫产产后出血中的临床效果

摘　　要：目的分析中央性前置胎盘孕妇剖宫产产后出血给予子宫压迫缝合联合宫腔球囊压迫治疗的临床效果。方法选取62例中央性前置胎盘剖宫产产后出血孕妇,依据治疗方法不同分为对照组(采用子宫压迫缝合术治疗)与研究组(采用子宫压迫缝合联合宫腔球囊压迫治疗),每组31例。比较两组手术相关指标、术后恢复指标、应激指标变化及并发症发生情况。结果相比对照组,研究组术中、术后24h出血量较少,术中止血时间、手术时间较短(P<0.05)。研究组术后血性恶露持续时间、下床活动时间、术后住院时间及月经复潮时间均较对照组短(P<0.05)。术后1d,研究组的皮质醇、去甲肾上腺素及肾上腺素指标均低于对照组(P<0.05)。研究组并发症发生率为6.45%,低于对照组的25.81%(P<0.05)。结论中央性前置胎盘剖宫产产后出血给予子宫压迫缝合联合宫腔球囊压迫治疗,手术相关指标理想,能够减轻术后应激指标,降低并发症发生,利于患者好转及恢复,效果较为理想。Objective To analyze the clinical effect of uterine compression suture combined with balloon compression in the treatment of central placenta previa hemorrhage after cesarean section.Methods 62 pregnant women with hemorrhage after cesarean section of central placenta previa were selected and divided into control group(treated with uterine compression suture)and study group(treated with uterine compression suture combined with uterine balloon compression)according to different treatment methods,with 31 cases in each group.The changes of operation-related indexes,postoperative recovery indexes,stress indexes and complications were compared between the two groups.Results Compared with the control group,the blood loss during and 24 h after operation in the study group was less,and the duration of intraoperative hemostasis and operation was shorter(P<0.05).The duration of postoperative lochia,the time of getting out of bed,the time of hospitalization and the time of menstruation in the study group were shorter than those in the control group(,<0.05).The levels of cortisol,norepinephrine and epinephrine in the study group were lower than those in the control group 1 day after operation(P<0.05).The complication rate of the study group was 6.45%,which was lower than that of the control group(25.81%)(P<0.05).Conclusion Uterine compression suture combined with uterine balloon compression for the treatment of hemorrhage after cesarean section of central placenta previa has ideal surgical indicators,which can reduce postoperative stress indicators,reduce complications,and help patients improve and recover,with ideal results.
