稽留流产药物治疗后胚物残留药物保守治疗与清宫术的效果研究

摘　　要：目的研究稽留流产药物治疗后胚物残留药物保守治疗与清宫术的效果。方法选取66例稽留流产患者,依据随机数字表法分为对照组(采用米非司酮+米索前列醇治疗后进行清宫术)与试验组(采用米非司酮+米索前列醇保守治疗),各33例。比较两组患者胚物残留时间、阴道持续出血时间、复诊次数、40d转经情况、感染率及再次手术率。结果试验组胚物残留时间、阴道持续出血时间长于对照组,复诊次数多于对照组,40d转经率高于对照组(P<0.05)。试验组术后感染率、再次手术率稍高于对照组,但比较差异无统计学意义(P>0.05)。结论稽留流产药物治疗后,对排胎后B超检查提示宫腔内仍有20~30mm的胚物残留,选择继续药物保守治疗,因其非侵入性操作,对于保护子宫内膜及减少宫腔粘连并发症更具有优势,远期伤害性较小,患者依从性更高,耐受性更好,为药物治疗后仍存在胚物残留的治疗方法提供更多参考。Objective To study the effect of conservative treatment of embryo residue after drug treatment for missed abortion and the operation of clearing palace.Methods 66 missed abortion patients were selected and divided into control group(mifepristone+misoprostol followed by uterine ablation)and experimental group(mifepristone+misoprostol conservative treatment)according to random number table method,with 33 cases each.The residual time of embryo,the duration of vaginal bleeding,the number of follow-up visits,40 days of menstrual rotation,the infection rate and the rate of reoperation were compared between the two groups.Results The residual time of embryo and the continuous bleeding time of vagina in the experimental group were longer than those in the control group,the number of revisits was more than that in the control group,and the 40 day rotation rate was higher than that in the control group(,<0.05).The postoperative infection rate and reoperation rate of experimental group were slightly higher than that of control groups but there was no statistical significance(P>0.05).Conclusion After drug treatment for missed abortion,B-ultrasound examination after abortion indicated that there was still about 20,30 mm of embryo residue in the uterine cavity.Therefore,conservative drug treatment should be continued because of its non-invasive operation,which has more advantages in protecting endometrial and reducing complications of uterine adhesion,less long-term harm,higher compliance and better tolerance of patients.To provide more reference for the treatment of embryo residue after drug treatment.
