探讨丙泊酚复合瑞芬太尼麻醉在腹腔镜卵巢囊肿切除术中的应用及安全性分析

摘　　要：目的探讨丙泊酚复合瑞芬太尼麻醉在腹腔镜卵巢囊肿切除术中的效果和用药安全性。方法选取60例卵巢囊肿切除术患者,使用抽签法将患者分为两组,每组30例。对照组单用丙泊酚麻醉,研究组使用丙泊酚复合瑞芬太尼麻醉。比较两组患者不同时段的血流动力学指标、意识苏醒时间、语言应答时间、住院时间及不良反应发生情况。结果研究组在麻醉诱导后(T_(1))、气管插管即刻(T_(2))、气腹形成5min(T_(3))、手术结束时(T_(4))的心率(HR)和平均动脉压(MAP)水平均低于对照组(P<0.05)。研究组的意识苏醒时间、语言应答时间及住院时间均较对照组短(P<0.05)。研究组的不良反应发生率6.67%低于对照组的26.67%(P<0.05)。结论在腹腔镜卵巢囊肿切除术中使用丙泊酚复合瑞芬太尼可减少对患者血流动力学指标的影响,且安全性高。Objective To analyze the use and safety of propofol combined with remifentanil anesthesia in laparoscopic ovarian cyst resection.Methods 60 patients with ovarian cyst resection were selected and divided into two groups with 30 cases in each group by drawing lots.The control group was anesthetized with propofol alone and the study group was anesthetized with propofol combined with remifentanil.The hemodynamic indexes,awakening time,speech response time,hospital stay and the occurrence of adverse reactions were compared between the two groups.Results The HR and MAP levels in the study group were lower than those in the control group after anesthesia induction(T_(1)),immediately after tracheal intubation(T_(2));5 minutes after pneumoperitoneum formation(T_(3)),and at the end of surgery(T_(4))(P<0.05).The awakening time,speech response time and hospitalization time of the stxidy group were shorter than those of the control group(P<0.05).The incidence of adverse reactions in the study group was 6.67%lower than that in the control group 26.67%(P<0.05).Conclusion The combination of propofol and remifentanil in laparoscopic ovarian cyst resection can reduce the impact on the hemodynamic indexes of patients and is safe.
