自拟益气化瘀汤联合黄体酮治疗药物流产后阴道出血临床效果

摘　　要：目的:观察自拟益气化瘀汤联合黄体酮治疗药物流产后阴道出血患者的效果及安全性。方法:选择安阳市第三人民医院2020年2月至2022年2月期间收治的92例药物流产后阴道出血患者,依据就诊时的单双号将患者分为对照组和观察组,各46例。对照组患者给予黄体酮进行治疗,观察组患者给予自拟益气化瘀汤联合黄体酮治疗,观察两组患者临床治疗总有效率、出血控制情况、月经恢复情况、治疗前后激素水平及血小板聚集率变化情况。结果:观察组患者治疗总有效率为95.65%,高于对照组的80.43%,差异具有统计学意义(P<0.05);观察组患者阴道持续出血时间及月经复潮时间均短于对照组,阴道出血量及月经复潮量均少于对照组,差异具有统计学意义(P<0.05);治疗后两组患者的孕酮(P)、人绒毛膜促性腺激素(HCG)、血小板聚集率均有不同程度下降,雌二醇(E2)均有不同程度提高,且治疗后观察组患者的P、HCG、血小板聚集率均低于对照组,E2高于对照组,差异具有统计学意义(P<0.05);观察组患者药物不良反应发生率为4.35%,低于对照组的17.39%,差异具有统计学意义(P<0.05)。结论:自拟益气化瘀汤联合黄体酮治疗药物流产后阴道出血效果明显优于单独使用黄体酮治疗,能够快速止血,促进月经恢复正常,改善凝血功能及激素水平,同时还能够降低药物不良反应发生率,疗效确切且安全性较高。Objective To observe the efficacy and safety of self-made Yiqi Huayu decoction combined with progesterone in the treatment of vaginal bleeding after drug abortion.Methods A total of 92 patients with vaginal bleeding after drug abortion admitted to Anyang Third People's Hospital from February 2020 to February 2022 were selected.According to the odd and even number at the time of visit,the patients were divided into a control group and an observation group,with 46 cases in each group.The control group was treated with progesterone,and the observation group was treated with self-made Yiqi Huayu decoction combined with progesterone.The total effective rate of clinical treatment,bleeding control,menstrual recovery,hormone levels and platelet aggregation rate before and after treatment were observed in the two groups.Results The total effective rate of the observation group was 95.65%,higher than 80.43%of the control group,and the difference was statistically significant(P<0.05).The duration of vaginal bleeding and menstrual return time of the observation group were shorter than those of the control group,and the amount of vaginal bleeding and menstrual return were less than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the serum levels of progesterone(P)and human chorionic gonadotropin(HCG),platelet aggregation rate were declined indifferent degrees,and the serum levels of estradiol(E2)was increased in different degrees,and the serum levels of P,HCG,platelet aggregation rate in the observation group were lower than those of the control group,the serum levels of E2 was higher than that of the control group,the differences were statistically significant(P<0.05);The incidence of adverse drug reactions in the observation group was 4.35%,lower than 17.39%in the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of self-made Yiqi Huayu decoction combined with progesterone in the treatment of vaginal bleeding after drug abortion
