中药膏摩联合脉冲超短波治疗慢性踝关节扭伤的临床研究

摘　　要：目的:研究中药膏摩联合脉冲超短波治疗慢性踝关节扭伤的临床效果。方法:选取2020年1月—2022年8月北京市昌平区中西医结合医院门诊慢性踝关节扭伤患者80例,随机分为治疗组和对照组,每组40例。治疗组采用中药膏摩联合脉冲超短波治疗,对照组采用双氯芬酸二乙胺乳剂外用配合脉冲超短波治疗。在治疗前及治疗后第3、7、14、28、42天进行视觉模拟评分量表(VAS)评分、Kofoed踝关节功能评分,并测量患踝软组织周径(四点定位法),对两组临床疗效进行比较。结果:治疗第3、7、14、28、42天,治疗组患者踝关节VAS评分降低值均大于对照组,Kofoed踝关节功能评分升高值均大于对照组,踝关节周径减小值均大于对照组,差异有统计学意义(P<0.05);治疗14d后,治疗组临床疗效优于对照组,差异有统计学意义(P<0.05)。结论:中药膏摩联合脉冲超短波治疗慢性踝关节扭伤,具备较好的疗效,能够有效减轻患者关节疼痛、缓解关节肿胀、改善关节功能。Objective To study the clinical effect of Chinese medicine paste mo combined with pulsed ultrashort wave in the treatment of chronic ankle sprains.Methods Eighty patients with chronic ankle sprains were selected from outpatient clinics of Beijing Changping District Integrated Hospital of Traditional Chinese and Western Medicine from January 2020 to August 2022,and were randomly divided into treatment group and control group,with 40 cases in each group.The treatment group was treated with Chinese herbal ointment mo with pulsed ultrashort wave therapy,and the control group was treated with diclofenac diethylamide emulsion topical application with pulsed ultrashort wave therapy.Visual Analogue Scale(VAS)score,Kofoed ankle function score,and measurement of soft tissue circumference of the affected ankle(four-point localisation method)were carried out before the treatment and on the 3rd,7th,14th,28th,and 42nd days after the treatment,and the clinical efficacy of the two groups was compared.Results On the 3rd,7th,14th,28th and 42nd day of treatment,the decrease of ankle VAS score in the treatment group was greater than that in the control group,the increase of Kofoed ankle function score in the treatment group was greater than that in the control group,and the decrease of ankle circumference in the treatment group was greater than that in the control group,the differences were statistically significant(P<0.05).After 14 days of treatment,the clinical efficacy of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusions Chinese medicine ointment massage combined with pulsed ultrashort wave in the treatment of chronic ankle sprain has a good curative effect,which can effectively reduce joint pain,relieve joint swelling and improve joint function.
