摘 要:目的:基于“脾开窍于口”探讨口腔微环境改变与大鼠胃幽门螺杆菌感染之间的相关性。方法:将30只雄性SD大鼠随机分为空白组(10只)、模型组(20只),空白组予以灭菌生理盐水灌胃,模型组予以常规标准幽门螺杆菌(Hp)菌液灌胃+口腔黏膜损伤并留少许菌液于口腔中,隔日1次,共6次。通过快速尿素酶检测法及HE染色法观察Hp定植及造模成功情况,采用酶联免疫吸附法(ELISA)检测血清、胃黏膜、口腔黏膜中TLR4、NLRP3的表达以及口腔黏膜与胃黏膜组织中Hp水平,采用Person相关性分析口腔黏膜和胃黏膜中TLR4、NLRP3、Hp水平的相关性。结果:胃Hp感染大鼠活动迟缓,反应迟钝,毛发疏松粗糙易脱落,食欲下降,精神不振,大便干稀。HE染色可见模型组大鼠口腔黏膜以及胃黏膜损伤明显;ELISA结果显示,模型组血清及口腔黏膜、胃黏膜中TLR4、NLRP3、Hp表达水平明显高于空白组(P<0.01);口腔黏膜与胃黏膜中TLR4、NLRP3、Hp水平呈正相关(P<0.05)。结论:口腔微环境改变是导致幽门螺杆菌反复感染的一个重要的因素,TLR4、NLRP3在大鼠胃幽门螺杆菌感染时表达上调,且口腔黏膜与胃黏膜中TLR4、NLRP3、Hp水平呈正相关。Objective:To explore the correlation between changes in oral microenvironment and helicobacter pylori infection in rats'stomach based on the concept of“spleen opening at the mouth”.Methods:Thirty male SD rats were randomized into a blank group(n=10)and a model group(n=20).The blank group was given sterilized physiological saline by gavage,while the model group was gavaged by routine standard helicobacter pylori(Hp)solution,the oral mucosa of the rats in this group was impaired,and a small amount of bacterial solution was left in the mouth,once every other day,for 6 times.The successful colonization and modeling of Hp were confirmed using rapid urease detection and HE staining methods.Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression of TLR4 and NLRP3 in serum,gastric mucosa,and oral mucosa,as well as the level of HP in oral mucosa and gastric mucosa.Person correlation analysis was employed to investigate the correlation of TLR4,NLRP3,and HP levels between in oral and gastric mucosa.Results:Rats infected with gastric Hp exhibited delayed activity,slow response,loose and rough hair,decreased appetite,decreased energy,and sometimes dry and sometimes loose stools.HE staining showed significant damage to the oral and gastric mucosa of the model group rats,and ELISA results showed that the expression levels of TLR4,NLRP3,and HP in the serum,oral mucosa,and gastric mucosa of the model group were significantly higher than those of the blank group(P<0.001).There is a positive correlation in the levels of TLR4,NLRP3,and HP between the oral mucosa and gastric mucosa(P<0.05).Conclusion:The alteration of oral microenvironment is an important factor leading to recurrent infection of helicobacter pylori,TLR4 and NLRP3 are upregulated during helicobacter pylori infection in rat stomach,and there is a positive correlation between the levels of TLR4,NLRP3,and HP in oral and gastric mucosa.
