谯凤英运用通窍活血汤加减结合针刺治疗鼻窒验案二则

摘　　要：鼻窒首见于《黄帝内经》,以经常性鼻塞为主要表现,相当于西医学中的慢性鼻炎,是耳鼻喉科常见的难治性疾病之一。导师谯凤英教授是天津中医药大学第一附属医院耳鼻喉科主任,硕士生导师,第七批全国老中医药学专家学术经验继承工作指导老师,从事耳鼻喉科临床、教学、科研工作40余载,博采众长,融会贯通,谙熟中医辨证论治,善用经方,临床经验丰富。谯师认为本病病位在鼻,病因多为病程日久,经久不愈,邪毒久留不去,壅堵鼻窍,气停血滞,而致鼻窍不通。治疗上根据鼻塞不通之症,通过辨病与辨证相结合,从“瘀”立论,以行气活血、化瘀通窍为原则,选用通窍活血汤加减结合针刺治疗以减轻鼻塞症状。通窍活血汤出自于王清任《医林改错》,是治疗血瘀证的经典古方,具有活血化瘀、通窍活络功效。针刺治疗选用针刺蝶腭神经节及传统鼻部选穴两法,均具有疏通鼻部经络、宣通局部气血、活血通窍止涕作用。通过针药合用,双管齐下,临床疗效甚佳。文章撷取谯凤英主任医师治疗鼻窒常见医案,并对其进行分析研究,以探讨临床辨治鼻窒经验,从“瘀”论治鼻窒及针药结合治疗鼻塞的可行性,为临床提高诊疗疾病效果提供新思路、新方法。Nasal asphyxia was first seen in Huangdi Neijing(《黄帝内经》),with frequent nasal congestion as the main manifestation,which is equivalent to chronic rhinitis in Western medicine and is one of the common refractory diseases in otolaryngology.Professor QIAO Fengying is the director of the Department of Otolaryngology of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine,the master's tutor,and a instructor of the seventh batch of national TCM experts academic experience inheritance work has been engaged in the clinical,teaching and scientific research of otolaryngology for more than 40 years.The professor has learned from many doctors' strengths,masters TCM syndrome differentiation and treatment,and makes good use of classic prescriptions,with rich clinical experience.The professor believes that the disease is located in the nose,and the cause is mostly that the disease course is long,it does not heal for a long time,and the evil poison stays for a long time,blocking the nasal passages,and resulting in the Qi( 气) and blood stagnation,and causes the nasal passages to be blocked.In terms of treatment,according to the symptoms of nasal congestion,through the combination of disease differentiation and syndrome differentiation,from the theory of stasis,based on the principle of promoting Qi and blood circulation and dissolving blood stasis,the addition and subtraction of the Tongqiao Huoxue decoction( 通窍活血汤) combined with acupuncture treatment were selected to reduce the symptoms of nasal congestion.The Tongqiao Huoxue decoction comes from WANG Qingren's Yilin Gaicuo(《医林改错》),which is a classic ancient prescription for the treatment of blood stasis syndrome,has the effect of activating blood and removing blood stasis and activating the meridians.The acupuncture treatment uses acupuncture to puncture the sphenopalatine ganglion and the traditional nasal acupuncture method,both of which have the effect of dredging the nasal meridians,promoting local Qi and blo
