基于“伏阳”理论探讨多囊卵巢综合征所致不孕症

摘　　要：多囊卵巢综合征以排卵障碍、高雄激素血症及胰岛素抵抗为主要临床表现,其病因病机复杂,为导致妇女排卵障碍而不孕的最常见因素,严重影响育龄期妇女的身心健康及生活。现代医学对本病以对症治疗为主,但多囊卵巢综合征的发病机制尚未明确,对症治疗多为激素用药,治疗周期长且常伴随一定的不良反应和风险,应用存在一定局限性。中医学能够辨证论治,整体调理,对多囊卵巢综合征所致不孕症有较好的临床疗效,发挥中医药优势治疗本病逐渐受到患者青睐。“伏阳”为机体局部或整体因痰、湿、瘀等而化气不足,阳气困于内部则郁,同时五脏及气血津液的生理功能不得发挥而呈现病理状态。文章基于“伏阳”理论,从肝、脾、肾气血痰瘀等方面分析多囊卵巢综合征所致不孕症的病因病机,探讨多囊卵巢综合征所致不孕症的治疗,认为“伏阳”是多囊卵巢综合征所致不孕症的病机核心,该病本虚标实,治疗时应标本同治、先后天同调以调经种子,既体现了中医辨证论治的理论特色,又结合了患者的切实需求。从“伏阳”理论出发,可为多囊卵巢综合征所致不孕症的临床诊疗开拓思路,为广大患者带来福音,提高患者满意度。Polycystic ovary syndrome is mainly characterized by ovulation disorder,hyperandrogenemia and insulin resistance.Its etiology and pathogenesis are complex,and it is the most common factor leading to ovulation disorder and infertility,which seriously affects the physical and mental health and life of women of reproductive age.Modern medical treatment of this disease is mainly symptomatic treatment,but the pathogenesis of polycystic ovary syndrome is not clear,symptomatic treatment is mostly hormone medication,treatment cycle is long and often accompanied by certain adverse reactions and risks,the application has certain limitations.Traditional Chinese medicine can treat diseases based on syndrome differentiation and overall conditioning,which has good clinical effects on infertility caused by polycystic ovary syndrome.The advantages of traditional Chinese medicine to treat this disease are gradually favored by patients.Latent Yang( 阳) is the deficien of Qi( 气) in the partial or overall body due to phlegm,dampness,blood stasis and other insufficient Qi,Yang trapped in the internal depression,at the same time,the five viscera and the physiological function of Qi,blood and body fluid are not allowed to play,resulting in a pathological state.Based on the theory of latent Yang,this paper analyzes the etiology and pathogenesis of infertility caused by polycystic ovary syndrome from the aspects of Qi,blood,phlegm and blood stasis in the liver,spleen,kidney,and probes into the treatment of infertility caused by polycystic ovary syndrome.It is believed that latent Yang is the core of pathogenesis of infertility caused by polycystic ovary syndrome.The disease is characterized by the mixture of deficiency and excess,and the treatment should be based on the principle of treating both the symptoms and root causes to adjust both congenital factor and acquired factor,so as to regulate menstruation and promote pregnancy,which not only reflects the theoretical characteristics of TCM syndrome differentiation and treatment,but a
