自拟解毒凉血汤联合西药治疗寻常型银屑病(血热证)的临床疗效及对生活质量的影响

摘　　要：目的:观察自拟解毒凉血汤联合西药治疗寻常型银屑病(血热证)的临床疗效及对患者生活质量的影响。方法:将80例寻常型银屑病(血热证)患者随机分为观察组和对照组,每组40例。对照组予以阿维A胶囊20~30 mg/d,卡泊三醇轻柔均匀涂抹于皮损处,2次/d,治疗4周。观察组加用自拟解毒凉血汤,药用生槐花15 g,紫草20 g,生地黄30 g,牡丹皮20 g,白茅根15 g,土茯苓15 g,金银花15 g,白鲜皮15 g,水牛角30 g,板蓝根15 g,龙骨(先煎)30 g,牡蛎(先煎)30 g,远志15 g,由医院统一制成装袋,200 mL/袋,1袋/次,2次/d,治疗4周。比较两组治疗后临床疗效、皮损面积和严重程度、炎性因子水平、瘙痒程度、生活质量评分。结果:治疗后,观察组总有效率高于对照组(P<0.05),炎性因子水平、银屑病皮损面积和严重程度评分、瘙痒评分、生活质量评分均低于对照组(P<0.05)。结论:自拟解毒凉血汤联合西药治疗寻常型银屑病(血热证)能提高临床疗效,改善临床症状,降低炎性反应,提高患者生活质量。Objective:To observe the clinical efficacy of self-prepared Jiedu Liangxue decoction(解毒凉血汤)plus western medicine on psoriasis vulgaris(blood-heat syndrome)and quality of life of patients.Methods:Eighty patients with psoriasis vulgaris(blood-heat syndrome)were randomly divided into the observation group and the control group,40 cases in eachO group.The control group was given 20-30 mg/d of acarb A capsules,gently and evenly applied with carpotriol on the lesion,twice a day,and treated for 4 weeks.The observation group was added with self-prepared Jiedu Liangxue decoction,with Sheng Huaihua(Flos Sophorae)15 g,Zicao(Radix Arnebiae)20 g,Sheng Dihuang(Radix Rehmanniae)30 g,Mudanpi(Cortex Moutan)20 g,Bai Maogen(Rhizoma Imperatae)15 g,Tu Fuling(Rhizoma Smilacis Glabrae)15 g,Jinyin Hua(Flos Lonicerae Japonicae)15 g,Baixianpi(Cortex Dictamni)15 g,Shuiniujiao(Cornu Bubali)30 g,Banlangen(Radix Isatidis)15 g,Longgu(Os Draconis)(decocted first)30 g,Muli(Concha Ostreae)(decocted first)30 g,Yuanzhi(Radix Polygalae)15 g,200 mL per bag,1 bag per time,2 times per day,and for 4 weeks.The clinical effect,lesion area and severity score,inflammatory factors,pruritus degree,and quality of life score were compared between the two groups after treatment.Results:After treatment,the total effective rate in the observation group was higher than the control group(P<0.05).The inflammatory factors,psoriasis lesion area and severity scores,pruritus score and quality of life score in the obseruation group were lower than the control group(P<0.05).Conclusion:The Jiedu Liangxue decoction plus western medicine on psoriasis vulgaris(blood-heat syndrome)can improve clinical efficacy,relieve clinical symptoms,reduce inflammatory response and improve quality of life of patients.
