摘　　要：腓总神经损伤在现代医学中属于下肢周围神经损伤疾病,占下肢神经损伤的50.9%,因其解剖位置表浅,故骨折、外伤、手术等均易致其损伤,在临床上多被忽视,常于患者出现症状后才被发觉。临床上尤以小腿及足运动功能和/或感觉功能异常为最明显的症状。随着病程的进展,患者可能出现足下垂、足背伸不能、足外翻功能障碍等,严重影响患者的日常生活。腓总神经损伤属于中医学中“痿证”范畴,指以肢体筋脉弛缓,软弱无力,不能随意运动,或伴有肌肉萎缩为主症的病证。本病病变部位在筋脉肌肉,病变脏器涉及肺、脾(胃)、肝、肾,基本病机为津液、气血、精髓亏虚,不能濡养肌肉筋脉。病理性质分为虚证与实证,也可见虚实夹杂。针灸在补虚泻实方面具有独特优势,而芒针相对于普通毫针具有刺激性更强、针感透传更深的特点,以此增强针灸治疗的效果。导师周婷主任为广德张氏芒针第四代传人,全国第五批名老中医继承人,从业20余年,尤擅长运用芒针治疗神经系统相关疾病,并在临床取得了良好的疗效。文章通过介绍周婷主任医师运用芒针治疗腓总神经损伤的医案,总结周婷主任治疗该病的临床经验,期望为临床治疗腓总神经损伤提供新的思路。Common peroneal nerve injury belongs to peripheral nerve injury of lower extremity in modern medicine,accounting for 50.9% of lower extremity nerve injury.Because of its superficial anatomical location,common peroneal nerve may be damaged by fracture,trauma,surgery and so on.It is easy to be ignored in clinical practice,and it is often found after patients have symptoms.Clinically,the most obvious symptoms are abnormal motor function and/or sensory function of the leg and feet.With the progression of the disease,patients may suffer from foot drop,inability of foot dorsalia,and foot valgus dysfunction,which seriously affects the daily life of patients.Common peroneal nerve injury belongs to the category of “paralysis syndrome” in TCM,which refers to the disease which is mainly characterized by relaxation of limbs,weakness,inability to exercise freely,or muscle atrophy.The pathological parts of this disease are in the muscles and and veins,and the diseased organs involve the lungs,spleen(stomach),liver and kidney.The basic pathogenesis is the deficiency of body fluid,Qi(气) and blood,and the essence,which cannot nourish the muscles and veins.The pathological properties can be divided into deficiency syndrome,excess syndrome and syndrome of intermingled deficiency and excess.Acupuncture and moxibustion has unique advantages in supplementing deficiency and draining excess.The elongated needle has the characteristics of stronger irritation and deeper penetration compared with ordinary milliform needles,so as to enhance the effect of acupuncture and moxibustion treatment.The author's mentor ZHOU Ting is the fourth generation successor of of Guangde ZHANG's elongated needle,one of the fifth batch of veteran TCM practitioner heirs,and has been practicing for more than 20 years,is especially good at applying elongated needle in the treatment of nervous system related diseases,and has achieved good clinical effects.This paper introduces the medical cases of Chief Physician ZHOU Ting in the treatment of common peroneal
