摘　　要：目的:研究超声窦卵泡计数(AFC)对不同年龄段多囊卵巢综合征(PCOS)的诊断性能及截断值,为临床诊疗工作提供具有针对性的参考意见。方法:选取2017年12月—2023年6月期间在梧州市红十字会医院妇科和生殖门诊诊断为PCOS的患者181例记为PCOS组,同期检查的181例健康妇女设为对照组。根据年龄将两组患者进一步分成4个亚组:<25岁组、25~29岁组、30~34岁组和≥35岁组。对所有患者行阴道超声卵巢AFC,并比较每个亚组内PCOS组与对照组AFC的差异性,绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),根据约登指数确定每个亚组AFC诊断PCOS最佳截断值。结果:每个亚组里PCOS组的AFC均高于对照组。<25岁组、25~29岁组、30~34岁组和≥35岁组AFC检测AUC均大于0.8,AFC诊断PCOS的截断值分别为17个、16个、14个、13个。结论:AFC对不同年龄段PCOS患者均有较高的诊断性能。AFC诊断PCOS的截断值随着年龄的增长而减小,制定截断值时应考虑年龄特异性。Objective To investigate the diagnostic efficacy and truncation value of antral follicle count(AFC) for polycystic ovary syndrome(PCOS) in different age groups,and to provide reference for clinical diagnosis and treatment.Methods 181 patients diagnosed with PCOS at the Obstetrics and Reproductive Clinic of the Red Cross Hospital in Wuzhou City from December 2017 to June 2023 were selected as the PCOS group,and 181 healthy women who underwent examinations during the same period were selected as the control group.The two groups were further divided into four subgroups according to age:< 25 years,25-29 years,30-34 years,and ≥35 years.Ovarian AFC was counted by vaginal ultrasonography in all patients,and the difference of AFC between PCOS group and control group in each subgroup was compared.Receiver operating characteristic(ROC) curve was drawn,area under the curve(AUC) was calculated,and the best cutoff value of AFC in diagnosing PCOS in each subgroup was determined according to Jorden index.Results AFC in PCOS group was higher than that in control group in each subgroup.The AUC for AFC detection in < 25 years old,25-29 years old,30-34 years old and ≥35 years old were all greater than 0.8,and the cutoff values for AFC diagnosis of PCOS were 17,16,14 and 13,respectively.Conclusion AFC has high diagnostic performance in PCOS patients of different ages.The cutoff value for AFC diagnosis of PCOS decreases with age,and age specificity should be considered when formulating the cutoff value.
