六味地黄汤治疗细菌性阴道病的疗效及对阴道菌群和免疫功能的影响

摘　　要：目的 观察六味地黄汤治疗细菌性阴道病的疗效及对阴道菌群和免疫功能的影响。方法 选取2021年10月—2023年10月于泉州市中医院门诊就诊的细菌性阴道病患者95例,依照双盲法将患者随机分为试验组47例和对照组48例。对照组采用替硝唑栓治疗,试验组在对照组治疗的基础上采用六味地黄汤治疗,7 d为1个疗程,2组均治疗3个疗程。比较2组治疗效果、临床症状消失时间,治疗前后阴道菌群、阴道微生态情况及免疫功能指标。结果 试验组总有效率为95.74%,高于对照组的81.25%(χ^(2)=4.873,P=0.027);试验组白带异常、外阴瘙痒、阴道疼痛消失时间短于对照组(P<0.01)。治疗3个疗程后,2组阴道乳杆菌数量及阴道pH值≤4.5者占比和乳杆菌占优势者占比高于治疗前,肠杆菌、类杆菌及支原体、白色念珠菌数量少于治疗前,白介素-12、γ-干扰素水平低于治疗前,且试验组变化幅度大于对照组(P<0.01)。结论 六味地黄汤治疗细菌性阴道病临床治疗效果较好,可加速症状缓解,平衡阴道内酸碱度,促进患者阴道内有益菌的生长,同时能够调节免疫功能,增强患者的抵抗力,加速疾病痊愈。Objective To observe the effect of Liuwei Dihuang soup in the treatment of bacterial vaginosis and its effect on vaginal flora and immune function.Methods Ninety-five patients with bacterial vaginosis who attended the outpatient clinic of Quanzhou Hospital of Traditional Chinese Medicine from October 2021 to October 2023 were selected,and they were divided into the experimental group(n=47) and the control group(n=48) according to the double-blind method.The control group was treated with tinidazole suppositories,and the experimental group was treated with Liuwei Dihuang soup on the basis of the treatment of the control group,with 7 d as a course of treatment,and both groups were treated for 3 courses of treatment.The clinical effect,symptom disappearance time,vaginal flora,vaginal microecology and immune function indexes before and after treatment were compared.Results The total effective rate of the experimental group was 95.74%,which was higher than that of the control group,which was 81.25%(χ^(2)=4.873,P=0.027);The disappearance time of leukorrhea abnormality,vulvar itching and vaginal pain in the experimental group were shorter than those of the control group(P<0.01).After 3 courses of treatment,the number of vaginal Lactobacillus and the percentage of those with vaginal pH≤4.5 and the percentage of those with Lactobacillus predominance in the 2 groups were higher than those before treatment,the number of Enterobacteriaceae,Bacteroidetes,and Mycoplasma,and Candida albicans were less than those before treatment,the levels of interleukin-12 and γ-interferon were lower than those before treatment,and the magnitude of the changes in the experimental group was greater than that in the control group(P<0.01).Conclusion Liuwei Dihuang soup treatment of bacterial vaginosis clinical treatment effect is good,can accelerate the relief of symptoms,balance the vaginal pH,promote the growth of beneficial bacteria in the patient vagina,and at the same time can regulate the immune function,enhance the patient's resistance
