地佐辛复合舒芬太尼对剖宫产产妇应激反应、肌肉松弛程度及牵拉反应的影响

摘　　要：目的探讨地佐辛复合舒芬太尼对剖宫产产妇应激反应、肌肉松弛程度及牵拉反应的影响。方法选取2020年1月至2022年1月收治的200例剖宫产产妇作为研究对象,根据麻醉方式不同将其分为对照组和观察组,每组100例。对照组采用舒芬太尼腰硬联合麻醉,观察组在对照组基础上加用地佐辛。比较两组的麻醉效果。结果手术10 min、术毕,观察组的皮质醇(Cor)、去甲肾上腺素(NE)及内皮素-1(ET-1)水平均低于对照组,差异具有统计学意义(P<0.05)。观察组的麻醉后肌肉松弛程度优良率高于对照组,差异具有统计学意义(P<0.05)。探查清理腹腔时,观察组的牵拉反应等级优于对照组,差异具有统计学意义(P<0.05)。观察组的不良反应总发生率低于对照组,差异具有统计学意义(P<0.05)。结论地佐辛复合舒芬太尼可以减轻剖宫产产妇的术中应激反应,提高肌肉松弛程度,减轻牵拉反应,且安全性更高,值得临床推广和应用。Objective To investigate the effects of dezocine combined with sufentanil on stress response,muscle relaxation degree and traction response in parturients undergoing cesarean section.Methods A total of 200 parturients undergoing cesarean section admitted from January 2020 to January 2022 were selected as the research objects and divided into control group and observation group according to different anesthesia methods,with 100 cases in each group.The control group was given sufentanil combined spinal epidural anesthesia,and the observation group was given dezocine on the basis of the control group.The anesthetic effects of the two groups were compared.Results At 10 min after operation and at the end of operation,the levels of cortisol(Cor),norepinephrine(NE)and endothelin-1(ET-1)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of muscle relaxation degree after anesthesia in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).When exploring and cleaning the abdominal cavity,the traction reaction level of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Dezocine combined with sufentanil can reduce the intraoperative stress response of cesarean section parturients,improve the muscle relaxation degree,reduce the traction response,and has higher safety,which is worthy of clinical promotion and application.
