行气活血消肿汤结合中医推拿手法治疗胫腓骨骨折后肢体肿胀的效果及对炎性因子水平的影响

摘　　要：目的探讨行气活血消肿汤结合中医推拿手法治疗胫腓骨骨折后肢体肿胀的效果及对炎性因子水平的影响。方法选取2021年3月至2022年4月收治的90例胫腓骨骨折后肢体肿胀患者为研究对象,根据随机数字表法将其分为对照组和观察组,各45例。对照组采用常规治疗+中医推拿手法,观察组在对照组基础上加行气活血消肿汤治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的肢体肿胀度、视觉模拟评分(VAS)评分低于对照组(P<0.05)。治疗后,观察组的肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)、单核细胞趋化蛋白-1(MCP-1)水平低于对照组(P<0.05)。治疗后,观察组的全血高切黏度、全血中切黏度、全血低切黏度及血浆黏度低于对照组(P<0.05)。结论行气活血消肿汤结合中医推拿手法治疗胫腓骨骨折后肢体肿胀的临床效果明显,可有效消除肢体肿胀程度,缓解疼痛,降低炎性因子水平,促进患肢血液循环,值得推广。Objective To investigate the effect of Xingqi Huoxue Xiaozhong decoction combined with traditional Chinese medicine massage on limb swelling after tibiofibular fracture and its influences on inflammatory factors levels.Methods A total of 90 patients with limb swelling after tibiofibular fracture admitted from March 2021 to April 2022 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 45 cases in each group.The control group was treated with routine treatment+traditional Chinese medicine massage,and the observation group was treated with Xingqi Huoxue Xiaozhong decoction on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the limb swelling degree and Visual Analogue Scale(VAS)score of the observation group were lower than those of the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),procalcitonin(PCT)and monocyte chemotactic protein-1(MCP-1)in the observation group were lower than those in the control group(P<0.05).After treatment,the whole blood high shear viscosity,whole blood middle shear viscosity,whole blood low shear viscosity and plasma viscosity in the observation group were lower than those in the control group(P<0.05).Conclusion Xingqi Huoxue Xiaozhong decoction combined with traditional Chinese medicine massage in the treatment of limb swelling after tibiofibular fracture has obvious clinical effect,it can effectively eliminate the degree of limb swelling,relieve pain,reduce the levels of inflammatory factors,and promote the blood circulation of affected limbs,which is worth promoting.
