应用胰岛素泵持续皮下输注门冬胰岛素短期强化治疗糖尿病的效果观察

摘　　要：目的:探讨应用胰岛素泵持续皮下输注门冬胰岛素短期强化治疗糖尿病的效果.方法:选取2022年10月—2023年10月南宁市宾阳县中医医院收治的糖尿病患者400例为研究对象,随机分为观察组与对照组,各200例.观察组给予胰岛素泵持续皮下输注门冬胰岛素短期强化治疗,对照组给予多次皮下注射胰岛素治疗.比较两组患者每日胰岛素用量、血糖达标时间、低血糖发生率、血糖水平、胰岛素抵抗指数、炎性因子水平.结果:观察组患者每日胰岛素用量少于对照组,血糖达标时间短于对照组,低血糖发生率低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者餐后2 h血糖、胰岛素抵抗指数、超敏C反应蛋白、肿瘤坏死因子-α水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05).结论:糖尿病患者应用胰岛素泵持续皮下输注门冬胰岛素短期强化治疗的效果较好,有助于减少胰岛素用量,快速稳定血糖水平,降低低血糖发生率及炎性因子水平.Objective:The study is performed to examine the effect of short-term intensive treatment of diabetes mellitus by applying insulin pump continuous subcutaneous infusion of insulin.Methods:400 cases of diabetic patients admitted to Binyang County Hospital of Traditional Chinese Medicine in Nanning from October 2022 to October 2023 were selected as the study subjects,and randomly divided into the observation group and the control group,200 cases each.The observation group was given short-term intensive treatment with continuous subcutaneous Insulin infusion by insulin pump,and the control group was given multiple subcutaneous insulin injections.The daily insulin dosage,time in glucose target range,incidence of hypoglycemia,blood glucose level,insulin resistance index,and inflammatory factor level of patients were compared in both groups.Results:The daily insulin dosage of patients in the observation group was less than that of the control group,the time in glucose target range is shorter than that of the control group,and the incidence of hypoglycemia was lower than that of the control group,and the difference was statistically significant(P<0.05);after treatment,2 h postprandial blood glucose,insulin resistance index,ultrasensitive C-reactive protein,tumor necrosis factor-αlevels of both groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:The application of short-term intensive therapy with continuous subcutaneous Insulin infusion by insulin pump in diabetic patients is more effective,which helps to reduce insulin dosage,rapidly stabilize blood glucose level,and reduce the incidence of hypoglycemia and the level of inflammatory factors.
